Essential Functions

• Provides routine nursing care and services that support the care delivered by patients requiring long-term acute or transitional restorative care in accordance with the established nursing care procedures and as directed by the supervisor.
• Addresses patient and family needs under the direct supervision of licensed personnel.
• Discusses and communicates the care plan for each patient in collaboration with licensed personnel to address all identified needs.
• Maintains up-to-date documentation of care provided to ensure the integration of information for use by the healthcare team, to ensure quality care.

Qualifications

• Medical Assistant experience is preferred.
• Must be able to effectively communicate in English via verbal and written means.
• Must be able to maintain patient confidentiality and assist licensed nursing staff with ADLs, vital signs, etc.
• Certified Nursing Assistant and/or Patient Care Technician certification is preferred.
• BLS certification is required upon hire.
