Job Title: Case Manager – Home Care

Job Summary

The Case Manager is responsible for coordinating, monitoring, and evaluating home care services for an active caseload of 80–100+ patients. This role ensures high-quality care delivery, effective scheduling, authorization compliance, and strong communication with patients, families, caregivers, referral sources, and internal teams. Key Responsibilities

  • Manage and oversee a caseload of 80–100+ patients, ensuring continuity, quality, and retention of care.

  • Develop, monitor, and evaluate individualized home care plans in collaboration with patients, families, and caregivers.

  • Conduct routine follow-up calls with patients and/or family members to assess satisfaction, address concerns, and resolve issues promptly.

  • Review, interpret, and break down payer authorizations, ensuring services are delivered within approved hours and guidelines.

  • Monitor and address overutilization or underutilization of authorized service hours.

  • Schedule home health aides and ensure full shift coverage, including managing last-minute call-outs and securing appropriate replacements.

  • Build and maintain an on-call pool of caregivers in coordination with HR and recruitment teams.

  • Match patient needs with suitable caregivers based on skills, availability, and compatibility.

  • Communicate effectively with referral sources, case managers, and payers to support patient care and business growth.

  • Resolve and document grievances or complaints from patients, families, and caregivers.

  • Monitor caregiver performance, document personnel issues, and report concerns in accordance with company policy.

  • Maintain accurate and up-to-date patient and personnel documentation in compliance with State and Federal regulations.

  • Promote caregiver engagement, morale, and retention through strong communication and support.

  • Support team initiatives and contribute to departmental performance and financial goals.

  • Ensure compliance with company policies, procedures, licensing requirements, and quality assurance standards.

Qualifications

  • High school diploma or equivalent required.

  • Minimum 2–3 years of experience in home care, case management, or service coordination.

  • Demonstrated ability to manage high-volume caseloads in a fast-paced environment.

  • Experience with scheduling, staffing coverage, and authorization management.

  • Strong written and verbal communication skills.

  • Excellent organizational, problem-solving, and time-management abilities.

  • Ability to maintain confidentiality and handle sensitive information.

  • Proficiency with computer systems and electronic documentation.

  • Experience with HHA Exchange and EVV systems preferred.

  • Bilingual English/Spanish preferred.

  • Emotional and mental maturity to establish and maintain effective working relationships with patients, families, caregivers, referral sources, and administrative staff.
