Part Time Home Health Nurse (RN)

POSITION SUMMARY: In accordance with an individualized plan of care, this position provides direct and practical medical care to older adults and person with disability who can't perform these tasks independently. Furthermore, performs medically prescribed skilled services requiring the application of the physical and social sciences and nursing skills in the care of the ill and/or disabled, or the prevention of disease and the conservation of health. This care is provided in accordance with Home Health Care plus policies and procedures and Indiana state nurse practice act.
EXPECTED RESULTS: The efforts of this position should result in:
1. The medical condition of 90% of consumers served is maintained or improved.
2. All consumer chart entries and reports are accurate and filed within 48 hours.
3. 98% of Consumers served maintain or increase their independence.
4. Participate in all required in-service training programs.
5. The health care needs of 98% of Consumers served are met.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Service Delivery- All service will be provided in accordance with the Plan of Care.
1. Make the initial evaluation visit when appropriate.
2. Regularly reevaluate the patient's nursing needs and give skilled nursing care to patients utilizing all aspects of the nursing process.
3. Initiate the plan of care and necessary revisions as ordered by the patient's physician.
4. Initiate appropriate preventive and rehabilitative nursing procedures.
5. Prepare clinical notes.
6. Coordinate services.
7. Inform the physician and other appropriate medical personnel of changes in the patient's condition and needs, counsel the patient and family in meeting nursing and related needs, participate in in-service programs, and supervise and teach other nursing personnel.
8. Accept and carry out physician, chiropractor, podiatrist, dentist, and optometrist orders (oral and written).
9. Assist the physician, chiropractor or podiatrist in evaluating level of function.
10. Initiates and applies appropriate preventive, therapeutic, and rehabilitative nursing procedures in accordance with the plan of care.
11. Teaches patient and significant other(s) as appropriate to assist the patient to attain the highest level of independent function possible.
12. Completes a comprehensive assessment as frequently as warranted by change(s) in the patient's condition but at least every 60 days, and makes recommendations for changes in the plan of care as necessary, under the physician's direction. Promptly informs the physician of any change in the patient's condition which suggests the need to alter the plan of care.
13. Assists the family to accept appropriate institutional means of care when warranted. Interprets extent and limitations of services available from the League.
14. Identifies the need for other therapeutic disciplines and initiates appropriate referral after consultation with the Director of Nursing and with the approval of the patient's physician.
15. Maintains awareness of community resources and utilizes them appropriately to meet patients needs.
Public Relations/Marketing
1. Fosters positive image of The League and Home Health Care Plus in the community-at-large.
2. Participate in public education and awareness activities.
3. Maintain positive relationships with consumers, community service agencies, government entities, co-workers, and other professionals.
Administrative and Clerical
1. As appropriate completes OASIS documents. Prepares timely, accurate and concise clinical notes and, when indicated, progress notes and summary reports. Prepares and maintains all clinical record documentation in a manner that demonstrates adherence to Agency policies and procedures and substantiates the Leagues claim for reimbursement.
2. Perform clerical duties associated with the position.
3. Accepts on-call responsibilities on a regular (rotating) basis.
Planning
1. Assists in the development of the plan of care.
2. Plans with family and physician for care which is within physical, financial, and emotional resources of the family.
3. Plans for coordinated nursing service with other therapeutic disciplines.
Personnel
1. Delegate duties and tasks to licensed practical nurses and other individuals as appropriate.
2. Direct the activities of the licensed practical nurse.
3. Participates in interdisciplinary conferences and staff meetings as needed.
4. Instructs and supervises the Home Health Aide, following Agency policy, to assure that goals for home health aide services are met. Prepares and updates the home health aide plan of care as often as required.
5 In the event the Agency hires Licensed Practical Nurses, the Home Health Nurse determines when skilled nursing care can be safely provided by the Licensed Practical Nurse (LPN) and provides instruction as needed. Provides supervision as often as needed but at least monthly supervises the LPN.
6. Participates in in-service and continuing education activities to update professional skills and knowledge.
7. Maintain a positive work environment;
8. Upon the request of the Director of Nursing/designee assist in the orientation of new employees
9. Upon the request of the Director of Nursing/designee participate in clinical record reviews and assists with the completion of other activities relative to performance improvement and the evaluation of nursing and home health aide services.
SUPERVISORY RESPONSIBILITIES: On a case by case basis, supervises the Home Health Aides providing care to patients assigned to the Home Health Nurse. Carries out all supervisory activities in accordance with the Leagues policies, procedures and applicable laws.
QUALIFICATIONS, REQUIREMENTS and DEMANDS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. EDUCATION and/or EXPERIENCE: Two (2) years of recent medical-surgical experience in acute or long-term care facility, rehabilitation or home health care setting. Two-year degree in nursing.
KNOWLEDGE REQUIREMENTS: Must have knowledge of nursing process, principles, and procedures.
LANGUAGE SKILLS: Ability to read, analyze, and interpret common professional journals, policy and procedure manuals, and business and professional letters. Ability to respond to common inquiries or complaints from customers. Ability to write reports that are complete, accurate and correct, and conform to prescribed style and format. Ability to effectively present information to management.
MATHEMATICAL SKILLS: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions and decimals. Ability to calculate figures and amounts, such as percentages and ratios.
REASONING ABILITY: Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions and deal with several abstract and concrete variables. Ability to exercise independent judgment.
INTERPERSONAL SKILLS: Ability to work effectively with other members of the health team and patients/significant others with varied socioeconomic backgrounds. Ability to establish
and maintain effective working relationships with all segments of the staff, professional community and community-at-large.
PHYSICAL DEMANDS: While performing the essential functions of this of this job the employee is required to: 1. regularly talk and hear to communicate, 2. regularly use fingers, hands, and arms to manipulate objects and conduct examinations, 3. frequently sit, stand, and walk, 4. occasionally lift and/or move up to 50 pounds and, 5. use close vision skills.
EQUIPMENT OPERATION REQUIREMENTS: Ability to independently operate, or learn to operate TTY and voice phones. Ability to independently operate, or learn to independently operate League office equipment such as fax and copy machines in order to perform job related duties and assignments. Ability to independently operate, or learn to independently operate the League’s computer network and software in order to perform job related duties and assignments.
TRAVEL REQUIREMENTS: Ability to travel independently: 1. frequently throughout the League’s service region.
WORK ENVIRONMENT: While performing the duties of this job, the employee is occasionally exposed to outside weather conditions. The noise level in the work environment is usually moderate.
WORK SCHEDULE DEMANDS: Must be available for work during regularly scheduled business hours. Must be available evenings and weekends as work demands require. Ability to
function effectively during periods of increased work demands. Ability to function in an independent self-directed manner.
