PART TIME POSITION- Monday and Thursdays 3pm-8pm

Description

· Paraprofessionals provide services to individuals in their own homes and communities, who need assistance caring for themselves as a result of old age, sickness, disability, and/or other inflictions.

· Personal Care may include assistance with activities of daily living, housecleaning, laundry, meal preparation, transportation, companionship, and respite.

· Paraprofessionals are responsible for ensuring that service is delivered in a caring and respectful manner, in accordance with relevant Agency policies and industry standards.


Service Definition Community Networking services provide individualized day activities that support the waiver beneficiary’s definition of a meaningful day in an integrated community setting, with persons who are not disabled. If the beneficiary requires paid supports to participate/ engage once connected with the activity, Community Networking can be used to refer and link individual. This service is provided separate and/or the home of a service provider. These services do not take place in licensed facilities and are intended to offer the beneficiary the opportunity to develop and meaningful community relationships with non-disabled individuals. Services are designed to promote maximum participation in community life while developing natural supports within integrated settings. Community Networking services enable the beneficiary to increase or maintain their capacity for independence and develop social roles valued by nondisabled members of the community. As the beneficiary gain skills and increase community connections, service hours may face.


Community Networking services consist of:

· Participation in adult education (College, Vocational Studies, and other educational opportunities);

· Development of community-based time management skills;

· Community-based classes for development of hobbies or leisure/cultural interest: Volunteer Work:

· Participation in formal/informal association and/or community groups;

· Training and education in self-determination and self-advocacy;

· Using public transportation;

· Inclusion in a broad range of community settings that allow the beneficiary to make community connections;

· For children, staffing supports are covered to assist children to participate in day care/after school summer programs that serve typically developing children and are not funded by Day Supports;

· Payments for attendance at classes and conferences is also included.

· Payments for memberships can be covered when the beneficiary participates in and integrated class;

· Transportation, when the activity does not include staffing support and the destination of the transportation in an integrated community setting or a self-advocacy activity. Payments for transportation are an established per trip charge or mileage.

· Enter Date in a timely manner each day


Reporting Relationship

· Reports to QP, Director of Operation, The Owner

Physical and Mental Demands

· Good physical and mental health.

· Physical ability to stand, walk, use hands and fingers, reach, stoop, kneel, crouch, talk, hear and see.

· Mental fortitude and stability to handle stress.

· Physical and mental ability to drive a vehicle.


Qualifications/Education

High School Diploma/GED

Current Driver's License

Proper Vehicle Insurance Coverage


Training/Experience

Must be 18 years old or older

Medication Administration Training-Successful Completion

CPR Certification

First Aid Certification

HIPPA/OSHA Training Successful Completion

Client's Rights abuse and neglect
