Individualized Home Supports (IHS) With Training Professional
Work with a small caseload of clients on their skills of independence. Tasks can include but are not limited to, community integration (transporting individuals out into the community), meal planning and prepping, budgeting, paperwork assistance etc. You'll manage your schedule with your clients and your supervisor.
Integrated Community Supports (ICS) Residential
ICS is a supportive apartment program designed to assist individuals to live as independently as possible in their own apartment. Independent Living Placement (ILP) has 6 different ICS locations. 5 sites in St. Paul and 1 site in Minneapolis. Each location has an apartment that is designed as the staff office and the rest is where clients live. You'll work with individuals on their skills of independence. Tasks can include but are not limited to, community integration (transporting individuals out into the community), meal planning and prepping, budgeting, paperwork assistance etc. When you aren't working directly with a client, you'll be available to them in the staff office for client emergencies that can include needing to de-escalate situations.
Housing Stabilization Services (HSS) Case Manager
Work with a small caseload of individuals on finding and maintaining housing. Search and apply for subsidized housing on clients behalf. Assist with coordinating home modifications for places when needed. Coordinate moves for clients. Assist with budgeting/benefits planning so that individuals learn how to pay for their housing and are set-up for success. Assist individuals with continued coordination with their landlords so that they can maintain their housing.
Employment Support Specialist
Work with a small caseload of individuals on exploring options for employment, developing them for employment, finding employment and maintaining employment. Assist individuals in finding what they are passionate about when it comes to employment. Assist with resume building, interview prepping and assist them in finding suitable clothing for interviews. Assist with searching and apply for jobs. Once individuals have the job, help them in coordinating with Human Resources on accommodations and on-the-job training.
Moving Home Minnesota (MHM)/Relocation Service Coordination (RSC) Case Manager
Work with a small caseload of individuals on helping them discharge from institutions out into the community. Assist with the State Medical Review Team (SMRT) process, applying for Social Security, applying for Medical Assistance, aged 65 years and older and setting up Mnchoices assessments to get individuals screened for MA paid waivers. Assist in finding appropriate housing. Assist with home modifications or vehicle modifications to make homes and vehicles more accessible.
Mental Health Case Management (MHCM)
Work with a caseload of individuals on implementing effective treatment/interventions reflective of identified patient needs. Complete diagnostic assessments (DA), provide on-going therapy with clients, develop functional assessment (FA), individual community support plan (ICSP_ and an individual family community support plan (IFCSP). Refer clients to resources, services and in-formal supports. Monitor the effectiveness of plans.
