Occupational Therapist (OT) - PRN

ORGANIZATIONAL RELATIONSHIPS
  1. Reports to Director of Rehabilitation Services (DOR).
  2. Supervises Certified Occupational Therapist Assistants (COTA), and Rehab Techs.
RESPONSIBILITIES
  1. Screens, evaluates, and treats patients.
  2. Evaluate patient within two working days of physician referral.
  3. Develop effective treatment plans.
  4. Supervise Occupational Therapist Assistants and Rehab Techs in direct patient care and patient related activities.
  5. Treat patients according to a treatment plan approved by the attending physician.
  6. Communicate with supervisor and other clinical care team members regarding patient progress, problems, and plans.
  7. Participate as necessary in patient care conferences and Daily and Weekly Clinical Case Management Meetings as indicated.
  8. Participate in in-service training programs.
  9. Record clinical documentation according to accepted regulatory, corporate and professional guidelines.
  10. Record daily treatments and labor per corporate procedures.
  11. Establish and instruct caregivers in patient specific maintenance programs.
  12. Participate in discharge planning.
  13. Complete Evaluations/Reports/Discharges within 24 hours of receipt of Physician Order.
  14. Print Evaluations/Reports/Discharges within 24-48 hours to be filed.
  15. Complete Telephone orders immediately following all Evaluations, Recertification and Discharges.
  16. Complete G-Codes during 10th Treatment Encounter Note visit or before.
  17. Treatment Encounter Notes must be entered daily before the close of business.
  18. Daily Encounter Record billing completed before the close of business.
  19. Weekly Progress notes must be completed every 7 calendar days or every 5 treatment days.
OCCUPATIONAL THERAPIST
  1. Secure necessary durable medical equipment and adaptive equipment for patients to facilitate independence.
  2. Act as a clinical preceptor for affiliated Occupational Therapy or Occupational Assistant schools.
  3. Comply with state regulations and national standards as they apply to the practice of Occupational Therapy.
QUALIFICATIONS
  1. Physical and Sensory Requirements (with or without the aid of mechanical devices):
  2. Mobility (includes walking, good balance), reaching, bending, lifting, grasping, fine hand coordination, ability to see and hear, ability to read and write, ability to communicate with patients, family members, personnel, and ability to remain calm under stress.
  3. Graduate from an accredited School of Occupational Therapy.
  4. Current licensure as an Occupational Therapist in the state of current practice.
  5. Must be able to communicate in English, both verbally and in writing.
  6. Must possess problem-solving skills of the type and at a level necessary to accomplish the job.
  7. Must possess good communication skills and be able to relate professionally and positively to patients, patient’s family members, and facility staff
  8. Must meet all local health regulations and pass post-employment physical exam or drug screen if required.
  9. Must be capable of performing the responsibilities of this job, with or without reasonable accommodations.
