The Nurse will assess
patients' health needs, develop and implement care plans, and provide ongoing
support and monitoring to ensure elders receive high-quality, individualized
care.
Responsibilities:
- Assess elders health needs and develop and implement care plans
- Administer medications and treatments as prescribed
- Monitor vital signs and report any changes to the appropriate staff member
- Communicate with elders and families to provide education and support
- Collaborate with other healthcare professionals to coordinate care and ensure continuity of treatment
- Documentation using electronic medical records
- Participate in professional development opportunities to maintain and improve skills
- Other duties as assigned
Qualifications:
- Current state license
- Excellent communication and interpersonal skills
- Ability to work collaboratively with other healthcare professionals
- Strong problem-solving and critical thinking skills
