Now Hiring: Physical Therapist – Per Diem (Hospice Care)

Unified Care Hospice Texas is seeking a compassionate and experienced Physical Therapist (PT) to join our interdisciplinary team. In this rewarding per diem role, you’ll provide comfort-focused therapy to patients at the end of life—promoting mobility, relieving discomfort, and supporting independence and dignity through gentle, goal-oriented interventions.

What You’ll Do:

• Conduct physical therapy evaluations to assess mobility, safety, and comfort.
  
  

• Develop individualized care plans centered on pain relief, positioning, and fall prevention.
  
  

• Educate families on safe mobility techniques and equipment use.
  
  

• Collaborate with the care team to support patients’ evolving needs and goals.
  
  

• Focus on enhancing quality of life—not rehabilitation.
  
  

What We’re Looking For:

• Graduate of an accredited DPT or MPT program.
  
  

• Active PT license in the state of Texas.
  
  

• Minimum of 2 years’ experience, ideally in hospice, palliative care, or home health.
  
  

• Empathy, clinical skill, and a passion for making a difference.
  
  

• Valid driver's license and reliable, insured vehicle.
