Neurology APRN – Skilled Nursing & Rehabilitation (Post-Acute Care)

We are hiring an experienced Neurology Advanced Practice Registered Nurse (APRN) to join a team in a Skilled Nursing Facility (SNF) and post-acute rehabilitation setting. This role is ideal for a Neuro Nurse Practitioner who has hands-on neurology experience and can function independently while collaborating with a multidisciplinary care team.

Responsibilities:

Neurology Assessment & Management

• Perform comprehensive neurologic assessments for patients with stroke, traumatic brain injury, spinal cord injury, Parkinson’s disease, multiple sclerosis, and other neurologic conditions.
• Manage post-acute complications including neurogenic bladder, seizures, spasticity, pressure injuries, dysphagia, and cognitive impairment.

Skilled Nursing & Rehab Care Coordination

• Develop individualized treatment plans in coordination with physicians, physical therapy (PT), occupational therapy (OT), speech therapy (ST), case managers, and family members.
• Lead interdisciplinary rounds and support discharge planning in the SNF and rehab setting.

Medication Management

• Prescribe, titrate, and monitor neurologic medications such as antiepileptics, muscle relaxants, and psychotropics.
• Adjust treatment plans based on patient response and side effects.

Patient & Family Education

• Educate patients and families on neurologic conditions, recovery goals, medications, and rehabilitation strategies.

Transition of Care

• Ensure smooth care transitions between hospital, skilled nursing facility, rehab, and home.
• Complete required documentation for durable medical equipment (DME), home health, and outpatient neurology referrals.

Prevention & Safety

• Monitor for complications including infection, delirium, or DVT.
• Implement fall prevention, pressure injury prevention, and skin integrity protocols in the rehab and SNF setting.

Qualifications:

• Master’s or Doctoral degree in Nursing (MSN or DNP).
• Certified as an Adult-Gerontology Acute Care NP (AGACNP), Family Nurse Practitioner (FNP), or Adult Nurse Practitioner (ANP) with neurology experience.
• Active Florida APRN license.
• Preferred certifications: Certified Neuroscience Registered Nurse (CNRN), NIH Stroke Scale.
• Current BLS/ACLS certification.
• 2+ years of experience in neurology or neuro-rehab
• Skilled in EMR use, interdisciplinary rehab goals, and functional scoring tools (FIM, MDS).
