The Patient Care Coordinator is a vital member of the clinical care team, responsible for preparing patients for their visits by performing intake, triage, and rooming procedures. This role includes collecting vitals, reviewing medical histories, updating charts in the electronic health record (EHR), and supporting the provider and nurse with clinical tasks to ensure efficient, family-focused care. The ideal candidate is warm, attentive, and organized, with a strong commitment to pediatric health and a passion for creating positive patient experiences in a fast-paced environment. 

 

Qualifications: 

  • High school diploma or equivalent required 

  • Completion of a Medical Assistant training program preferred 

  • Current Nursing program student a plus 

  • Current CPR/BLS certification 

  • Experience in a pediatric or clinical setting preferred 

  • Proficiency in using electronic health record systems 

  • Ability to pass a background check and drug screening

Key Responsibilities: 

  • Greet and room patients in a friendly and professional manner 

  • Obtain and record vital signs including temperature, pulse, respiratory rate, blood pressure, height, head circumference, and weight 

  • Review and update patient medical histories, current medications, allergies, and chief complaints 

  • Communicate pertinent intake and triage information to providers before exams 

  • Prepare exam rooms by ensuring cleanliness, appropriate supplies, and readiness between patients 

  • Assist providers and nurses with procedures, testing, or screenings as needed 

  • Enter documentation accurately and promptly into the electronic health record 

  • Monitor and respond to patient needs during the visit to ensure comfort and safety 

  • Maintain clinical supply inventory and restock exam rooms 

  • Unpack shipments, including supplies, formula, samples, and vaccines and place in appropriate areas physically and in EHR 

  • Follow infection control protocols and safety procedures at all times 

  • Participate in team huddles, training sessions, and quality improvement initiatives 

  • Performs other duties that may be necessary or in the best interest of the organization
