Position Details:
· Grand Traverse Pavilions is seeking a full-time MDS Assessment Coordinator to oversee the federally mandated Resident Assessment Instrument (RAI) process.
· The base wage for this position is competitive and based on experience.
· Must be a Registered Nurse (RN) with MDS experience or certification (LPNs/LVNs may be considered for secondary MDS roles with RN oversight).
· Must be able to pass a criminal background check and drug screen.
Responsibilities:
· Oversee the RAI process, including MDS assessments, care area assessments, and care plan development/revisions.
· Coordinate and ensure compliance with federal and state regulations regarding MDS and care plans.
· Collaborate with the interdisciplinary team to complete MDS assessments and develop comprehensive care plans.
· Maintain the OBRA and PPS assessment schedules and ensure accurate documentation.
· Support Medicare eligibility determinations and manage skilled level of care assessments.
· Participate in QAA/QAPI committees and work with case managers for residents utilizing alternative payment models.
· Review and improve staff competency and education to ensure accurate MDS completion.
· Monitor MDS-based Quality Measures and maintain confidentiality of sensitive information.
· Collaborate with physicians and hospital discharge planners to ensure complete and accurate diagnosis coding.
Qualifications:
· Current RN license required (LPN/LVN may be considered for secondary roles).
· MDS experience or certification preferred.
· Knowledge of federal and state regulations, including Resident Assessments and Care Plans.
· Strong attention to detail, organizational, and time-management skills.
· Critical-thinking, problem-solving, and communication skills.
