LPN or RN to provide care to young child in their home
We are seeking a compassionate and dedicated LPN or RN to provide one-on-one in-home nursing care for an active, cheerful toddler in Rockford, IL (61107). The child is trach- and G-tube dependent, receiving all nutrition and medications through the G-tube. This role involves delivering skilled, attentive care while supporting a fun, engaging daily routine—whether watching Ms. Rachel and Lucas and Friends or enjoying playtime on the jungle gym.
Schedule: Monday–Friday, 7:00 a.m.–5:00 p.m.
Position Type: Full-time, in-home private duty nursing
Responsibilities:
Provide skilled nursing care according to the care plan
Monitor health status, track vital signs, and administer medications/treatments
Collaborate with family and care team to ensure the best outcomes
Maintain accurate charting and documentation
Requirements:
Current Illinois RN or LPN license
CPR required (Castor is able to assist with internal certification if needed)
Pediatric or young adult experience preferred (but not required)
Compassionate, reliable, and patient-centered approach
