Job Summary: The Licensed Practical Nurse (LPN) provides basic medical and nursing care to patients within a skilled nursing facility. The LPN works under the supervision of a registered nurse or physician and provides direct patient care such as medication administration, wound care, monitoring vital signs, and assisting with activities of daily living.

Key Responsibilities:

  • Administer medications and treatments as prescribed by the physician or registered nurse
  • Monitor and record vital signs, such as blood pressure, temperature, pulse, and respiration
  • Assist with the activities of daily living, including bathing, dressing, grooming, and toileting
  • Perform basic nursing procedures, such as catheterization, tube feedings, and wound care
  • Document patient care activities in the medical record accurately and in a timely manner
  • Communicate with physicians and other healthcare team members to ensure continuity of care
  • Participate in interdisciplinary team meetings to discuss patient care and treatment plans
  • Adhere to facility policies and procedures, including infection control, safety, and HIPAA regulations
  • Maintain a clean and organized work environment
  • Provide emotional support and education to patients and families

Qualifications:

  • Graduate of an accredited practical nursing program

  • Licensed OH LPN

  • Current CPR certification

  • Excellent communication and interpersonal skills

  • Ability to work independently and as part of a team

  • Strong organizational and time-management skills

  • Compassionate and empathetic towards patients and families

  • Ability to lift and move patients as needed
