Job Summary: The Licensed Practical Nurse (LPN) provides basic medical and nursing care to patients within a skilled nursing facility. The LPN works under the supervision of a registered nurse or physician and provides direct patient care such as medication administration, wound care, monitoring vital signs, and assisting with activities of daily living.
Key Responsibilities:
- Administer medications and treatments as prescribed by the physician or registered nurse
- Monitor and record vital signs, such as blood pressure, temperature, pulse, and respiration
- Assist with the activities of daily living, including bathing, dressing, grooming, and toileting
- Perform basic nursing procedures, such as catheterization, tube feedings, and wound care
- Document patient care activities in the medical record accurately and in a timely manner
- Communicate with physicians and other healthcare team members to ensure continuity of care
- Participate in interdisciplinary team meetings to discuss patient care and treatment plans
- Adhere to facility policies and procedures, including infection control, safety, and HIPAA regulations
- Maintain a clean and organized work environment
- Provide emotional support and education to patients and families
Qualifications:
- Graduate of an accredited practical nursing program
- Licensed OH LPN
- Current CPR certification
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong organizational and time-management skills
- Compassionate and empathetic towards patients and families
- Ability to lift and move patients as needed
