Summary

As a Charge Nurse at Reliant Care Management Company, you will play a vital role in overseeing patient care for our elderly residents. Responsibilities

• Supervise nursing staff and coordinate patient care activities.
• Assess patient needs and develop individualized care plans.
• Ensure compliance with healthcare regulations and standards.
• Provide direct patient care, including medication administration and monitoring vital signs.
• Educate patients and families on health management and treatment plans.
• Collaborate with physicians and other healthcare professionals to optimize patient outcomes.
• Maintain accurate documentation of patient care activities and progress.
• Foster a supportive environment that promotes teamwork and open communication.

Qualifications

• Valid Kansas nursing license (LPN) or (RN) with experience in geriatric or long-term care settings.
• Strong knowledge of infectious disease care protocols.
• Excellent patient care skills with a focus on empathy and dignity.
• Ability to lead and mentor nursing staff effectively.
• Familiarity with healthcare systems such as Point Click Care is preferred but not required.
• Strong communication skills for effective interaction with patients, families, and team members.
• Experience in developing care plans tailored to individual patient needs.
