• Assess residents’ health status and develop individualized care plans

• Administer medications and treatments as prescribed

• Monitor vital signs and report significant health changes

• Educate and support residents and their families

• Collaborate with interdisciplinary healthcare team members

• Maintain accurate documentation in electronic medical records

• Participate in ongoing training and professional development

• Current and active LVN license in the State of Texas

• Minimum of 1 year of nursing experience preferred

• Strong communication and interpersonal skills

• Reliable, team-oriented, and committed to resident-centered care
