We’re hiring a dependable, resident-focused LVN. Join a supportive team with strong clinical standards, growth opportunities, and immediate interviews.

What You’ll Do

Shift Leadership & Handoffs

  • Give/receive thorough shift report; review MAR/TAR, new orders, falls/skin/behavior notes, and isolation lists.

  • Set hallway priorities; assign and support CNAs; coach in real time; perform scheduled and PRN safety rounds; ensure rooms are admission-ready.

Medication & Treatment Administration

  • Complete med pass accurately and on time; verify the Five Rights; document in MAR/TAR/EMR.

  • Administer treatments per orders (dressings/wounds, trach/ostomy, G/PEG care/flushes/meds, catheter care/irrigation).

  • Manage PRN meds with timely reassessment; handle controlled-substance counts, wastes, and discrepancy reporting; reorder/dispose per policy.

Skilled Nursing (within LVN scope & training)

  • Urinary: insert/remove/irrigate catheters; obtain sterile specimens.

  • Respiratory: trach care, suctioning, oxygen, nebulizers, pulse-ox monitoring.

  • Enteral: G/J/PEG tube maintenance, feeding setup, flushes, meds.

  • Wound/Skin: advanced dressings, packing, NPWT (wound vac) as trained; suture/staple removal per order.

  • IV therapy if certified and per facility policy (peripheral start/site care; fluids/meds).

Clinical Monitoring & Escalation

  • Trend vitals, pain, behavior/sleep, I/O, and weights; identify changes in condition early.

  • Notify provider/NP/PA and family per policy; implement new orders; document all contacts and outcomes.

  • Initiate incident reports (falls, injuries, med variances); respond to emergencies (RRT/code); maintain code cart/oxygen checks.

Admissions, Returns & Discharges (Evenings/Nights)

  • Complete initial assessments, med reconciliation, orders entry, orientation, belongings inventory, and thorough documentation.

  • Coordinate labs, imaging, transport, pharmacy, and on-call providers as needed.

Documentation & Compliance

  • Chart timely and accurately in the EMR; ensure MAR/TAR accuracy, treatment records, and care-plan notes.

  • Maintain required clinical logs (falls, infection control, wounds, psychotropics, restraints) as directed; support survey readiness and POCs.

Infection Control & Safety

  • Follow standard/contact/droplet precautions; hand hygiene/PPE.

  • Complete temperature logs as assigned; disinfect equipment after use; implement fall-prevention and elopement protocols; ensure alarms/devices function.

Team Collaboration & Resident Experience

  • Partner with the DON/RN supervisor, therapy, social services, and dietary to meet resident goals.

  • Communicate clearly and compassionately with residents/families; provide updates and education.

  • Assist with ADLs as needed during lean staffing to maintain dignity and timeliness of care.

Environment & Equipment

  • Keep med/treatment carts secure, stocked, and sanitary; report malfunctioning equipment; maintain an organized nurse station and supply areas; complete end-of-shift checklists.

Qualifications

  • Active, unencumbered Texas LVN required

  • Current CPR/BLSIV Therapy certification preferred

  • Knowledge of nursing standards and SNF/LTC regulations; sound independent decision-making within scope

  • Leadership/supervisory ability; professional, tactful communication with residents, families, staff, providers, and regulators

  • EMR proficiency (PointClickCare a plus); excellent organization and follow-through

  • Able to stand/walk the shift and safely assist residents; lift 60–70 lbs as needed; uses proper body mechanics/PPE

  • Reliable attendance; completes required in-services/education; maintains HIPAA confidentiality

  • Meets all pre-employment/health requirements (background, drug screen, TB/clearances)
