Reports to: Director of Nursing
Job Summary: The Licensed Practical Nurse (LPN) provides basic medical and nursing care to patients within a skilled nursing facility. The LPN works under the supervision of a registered nurse or physician and provides direct patient care such as medication administration, wound care, monitoring vital signs, and assisting with activities of daily living.
Key Responsibilities:
· Administer medications and treatments as prescribed by the physician or registered nurse
· Monitor and record vital signs, such as blood pressure, temperature, pulse, and respiration
· Assist with the activities of daily living, including bathing, dressing, grooming, and toileting
· Perform basic nursing procedures, such as catheterization, tube feedings, and wound care
· Document patient care activities in the medical record accurately and in a timely manner
· Communicate with physicians and other healthcare team members to ensure continuity of care
· Participate in interdisciplinary team meetings to discuss patient care and treatment plans
· Adhere to facility policies and procedures, including infection control, safety, and HIPAA regulations
· Maintain a clean and organized work environment
· Provide emotional support and education to patients and families
Qualifications:
· Graduate of an accredited practical nursing program
· Active LPN license in the state of employment
· Current CPR certification
· Minimum of 1-year experience in a long-term care or skilled nursing facility preferred
· Excellent communication and interpersonal skills
· Ability to work independently and as part of a team
· Strong organizational and time-management skills
· Compassionate and empathetic towards patients and families
· Ability to lift and move patients as needed
Physical Demands:
The LPN must be able to lift and move patients (up to 50 pounds).

Pay: Up to $31.00 per hour
