Mobile Health Clinic LPN

Program and Site Location: Various Locations (Kamloops and Merritt)
Reports To: TiOAT Coordinator
Term: Full-Time Monday-Friday 8:30am-4:30pm
Wage Range: $32.84 up to $41.35 based on education and related experience.
Purpose and Function:
As the Mobile Health Clinic LPN, you will work in collaboration with Telus, the TiOAT Team, Interior Health, the Ministry of Health (MOH), the Mental Health and Addictions (MHA), and the Outreach Team to initiate and deliver the various services. The Mobile Medical Clinic Nurse operates in accordance with the competency guidelines and practices within the Standards of Practice as outlined by the College of Licensed Practical Nurses of British Columbia (BCCNM). The Mobile Health Clinic LPN will also work in collaboration with medical practitioners to assist with virtual health appointments and provide nursing assessments and interventions related to common diagnoses and specific clinical situations relevant to vulnerable populations. Ensures proper documentation and procedures are completed prior to the intake appointment, and performs nursing skills, including specimen acquisition in accordance with established policies, protocols, and procedures. Service delivery is based on a client-centered, non-judgmental perspective

Specific Responsibilities:
- Working side by side with a dedicated outreach support worker, the Nurse will provide assessments and interventions on a wide variety of issues, within their scope of practice.
- The Mobile Nurse will ensure their team follows a schedule to ensure clients in the community have access to treatment in a consistent manner.
- Connect clients to resources in the community.
- Working collaboratively with the outreach worker to provide clients with items to meet their basic needs
- Assist in arranging transport for clients in need of a higher level of care or transport to appointments through outreach services.
- Building trust and rapport within the community
- Collaborate with other members of the Nursing Team to ensure appointments at the Spero Clinic are arranged.
- The Nurse will confer with community groups/agencies to help reach as many clients as possible.
- Collaborate with the coordinator of TiOAT Services to make changes in the day-to-day planning for the Mobile Medical clinic as needed.
- Perform wound care whenever required

- Documents and reports all assessments, records observations, care provision, and client responses following policies, protocols, procedures, and standards, which may include computerized records and databases.
- Assist team with required verbal and written reports, case notes, letters, memoranda, and year-end reports.
- Maintain files and statistics as per accreditation standards.
- When requested, be a member of the Society’s Performance and Quality Improvement (PQI) team.
- Ensure compliance with all Society policies and procedures.
Qualifications and Suitability:

- Successful completion of a recognized program for Licenced Practical Nurses.
- Current full practicing licensure with the College of Licensed Practical Nurses of BC (BCCNM).
- Two years experience working with vulnerable adults, mental health/addiction/homelessness issues an asset.
- Comfortable around clients in active substance use including injection of substances.
- Strong ability to recognize and address difficult behaviors.
- Good communication, documentation, interpersonal, and conflict management skills.
- Ability to work within a team-based environment involving multi-agency support.
- Strong working knowledge of community resources.
- Understands and maintains professional boundaries.
- Valid driver’s license, clean driving abstract.
- Must pass an acceptable criminal record check for the vulnerable sector.
- Occupational First Aid Level 1 and CPR-C
- Readiness to complete in-house online training within the first month of employment.
- Mental Health First Aid, ASIST, and Trauma Informed Practice an asset.
