We are currently seeking a Full-Time Licensed Practical Nurse (LPN) to join our growing team. If you are passionate about making a difference in the lives of others while working in a supportive environment, we’d love to hear from you!


Responsibilities & Duties

As an LPN with Hand N Hand Home Healthcare, you will:

  • Provide skilled nursing care under the direction of an RN and physician orders.

  • Monitor, assess, and document patients’ conditions, including vital signs and progress reports.

  • Administer medications, treatments, wound care, and injections as prescribed.

  • Assist with patient ADLs (Activities of Daily Living) when needed.

  • Educate patients and families on care plans, treatments, and health maintenance.

  • Collaborate with the healthcare team to ensure continuity of care.

  • Maintain accurate, timely, and detailed documentation.

  • Participate in care planning, patient assessments, and case conferences.

  • Ensure compliance with state regulations, CMS/Medicare guidelines, and company policies.

  • Provide emergency interventions as necessary.

  • Demonstrate professionalism and uphold Hand N Hand’s values in every interaction.


Qualifications
  • Current Licensed Practical Nurse (LPN) license in the state of Tennessee.

  • Minimum 1 year of nursing experience (home health experience preferred).

  • Strong clinical assessment and documentation skills.

  • Compassionate, patient-centered approach to care.

  • Excellent communication and teamwork skills.

  • Valid driver’s license and reliable transportation.

  • CPR certification required.
