Licensed Practical Nurse

Position Summary
The Licensed Practical Nurse (LPN) provides high-quality nursing care to patients under the supervision of a Registered Nurse (RN) or physician. The LPN ensures that patients receive compassionate, professional, and effective care in accordance with the Connecticut Board of Nursing standards and facility policies. This role requires strong clinical judgment, attention to detail, and a commitment to promoting patient comfort and safety.
Key Responsibilities

Provide direct patient care in accordance with established nursing care plans and physician orders.

Administer medications and treatments safely and accurately.

Monitor, document, and report changes in patient condition to the supervising RN or physician.

Assist with wound care, vital signs, and specimen collection as directed.

Support patients with activities of daily living (ADLs) and promote independence when possible.

Ensure accurate and timely completion of nursing documentation in electronic health records (EHR).

Maintain infection control and safety standards per OSHA, CDC, and Connecticut Department of Public Health regulations.

Collaborate with interdisciplinary care teams, including physicians, therapists, and aides.

Educate patients and families on treatment plans, medication use, and health maintenance.

Participate in staff meetings, in-service training, and continuing education programs.
Qualifications-
Active and unrestricted Connecticut LPN license.

Graduation from an accredited Practical Nursing program.

Current CPR/BLS certification.

Minimum 1 year of nursing experience preferred (home health, long-term care, or clinical setting).

Strong communication, critical thinking, and organizational skills.

Ability to work independently and as part of a collaborative care team.

Must be available at least 3 days per week (with flexibility for evenings, weekends, or holidays as needed).
Preferred Skills-
Experience with EHR systems (e.g., Epic, PointClickCare, or HomeCare HomeBase).

Familiarity with state documentation and compliance requirements.
