LICENSED PRACTICAL NURSE

Our Skilled Nursing Care Facility is seeking motivated and compassionate Licensed Practical Nurses to join our growing staff and provide direct care to residents while promoting the highest level of functioning.

We are currently seeking Full-Time and Part-Time LPNs for ALL Shifts 3pm-11pm, 11pm-7am)

Responsibility:

• Monitoring residents, and their responses to treatment care plans.
• Supervising and directing.
• Document necessary information regarding nursing care, care plans, and observation of resident's overall conditions.
• Perform various duties to provide quality nursing care to residents.
• Analyzes information for appropriate interventions to prevent decline.
• Record signs of change in condition.
• Notify appropriate staff per facility protocol to evaluate weight loss, a decline in ambulation, development of skin breakdown, etc.
• Administers medications and treatments to residents; monitor administration of intravenous fluids; places orders for medication.
• Maintain resident clinical files; keep charts updated; document appropriately.
• Make rounds to see each resident daily.

Requirements

• Preferred to have experience in a Long-Term Care environment, but new grads are welcome!
• Must possess and maintain a current NYS state license as a Licensed Practical Nurse (LPN).
• Must be able to move, lift and position patients.
• Knowledge of Sigma Care is Preferred
