POSITION SUMMARY:
Reporting to the Registered Nurse (RN) on site, the Licensed Practical Nurse (LPN) helps make a difference in our community by providing care for residents at both Brentwood House and Suggett House. These two locations are state-of-the-art residential dementia care settings in Brentwood Bay that provide a home environment for seniors in need of specialized dementia care twenty-four hours a day. The LPN supports the delivery of compassionate, efficient, and quality care to support residents to maintain the highest quality of life possible. They perform all duties with the full scope of practices related to resident care. The LPN directly supervises care aides, assisting with training, evaluation, and performance management.
DUTIES AND RESPONSIBILITIES:
- Resident Care and Assessment: Conduct comprehensive assessments (e.g., RAI 2.0), develop individualized care plans, monitor resident vitals and health, provide personal care, and engage residents in daily activities and life events.
- Medication Management: Receive, process, and administer medications as per physician orders, assess medication use, and communicate resident needs to physicians.
- Team Leadership and Support: Provide leadership, supervision, and support to care aides; oversee the orientation and training of new employees and practicum students; and delegate daily tasks.
- Communication and Coordination: Facilitate admission processes, complete related paperwork, communicate with families and case workers, coordinate supplies and resident needs, and provide tours to potential residents and families.
- Safety and Compliance: Maintain up-to-date knowledge of relevant policies and procedures, support safety culture through incident reporting and corrective actions, adhere to COVID-19 policies, and stay current with annual competencies and education requirements
EDUCATION AND EXPERIENCE:
- Full-scope LPN Diploma
- Registered with the BC College of Nurses and Midwives
- Supervisory and Dementia Care experience preferred- at times works as the only nurse on the shift
- First Aid with CPR Certificate- designated First aid attendant on shift
- FOODSAFE Certificate
SKILLS AND ABILITIES:
- Strong Communication skills, both verbal and written
- Ability to build strong relationships with residents, families, co-workers, members of the multidisciplinary team and the public
- Ability to organize work and implement a plan of care
- Skilled at conflict resolution
- Demonstrated leadership skills
- Skilled in the use of Microsoft 365 (Outlook, Teams, Word, etc.)
- Ability to operate related equipment
- Physical ability to carry out the duties of the position
