Position Summary

The Licensed Master Social Worker (LMSW) – Case Manager provides comprehensive case management, care coordination, and psychosocial support to patients of Health Care Partners of South Carolina (HCPSC). This role works collaboratively with medical providers, behavioral health specialists, and community partners to address social determinants of health, improve patient outcomes, and promote overall wellness. The LMSW – Case Manager ensures that patients have access to appropriate medical, behavioral, and social services, while advocating for equitable care and empowering patients to actively participate in their health journey.

Essential Duties and Responsibilities

Conduct thorough biopsychosocial assessments to identify patient needs, strengths, and barriers to care.

Develop individualized care plans in collaboration with patients, families, and interdisciplinary care teams.

Coordinate referrals to internal and external resources, including housing, food assistance, behavioral health, substance use treatment, and vocational services.

Provide short-term counseling, crisis intervention, and supportive case management.

Advocate for patients within healthcare, insurance, and social service systems to ensure timely access to care.

Monitor patient progress, adjust care plans as needed, and document all interactions in the electronic health record (EHR) in compliance with FQHC and regulatory standards.

Facilitate patient education on health management, preventive care, and available community resources.

Participate in quality improvement initiatives, case conferences, and staff training.

Maintain confidentiality and adhere to HIPAA and all applicable laws and regulations.

Perform other duties as assigned to support organizational goals and compliance requirements.

Qualifications

Master’s degree in social work (MSW) from an accredited program.

Current LMSW licensure in South Carolina.

Minimum of 1–2 years of experience in case management, care coordination, or social services (FQHC, community health, or healthcare setting preferred).

Knowledge of community resources, public benefits, and social service systems.

Strong interpersonal, communication, and advocacy skills.

Ability to work effectively with diverse populations and in a multidisciplinary team environment.

Bilingual skills (English/Spanish or other languages) highly desirable.

Physical Requirements

Ability to sit, stand, and walk for extended periods.

Occasional lifting of up to 20 pounds.

Frequent use of computer, telephone, and other office equipment.

Ability to travel locally for home visits, community outreach, and partner meetings.

Visual and auditory acuity to perform job-related functions.

Technical Proficiency

Proficient in EHR systems (e.g., eClinicalWorks).

Skilled in Microsoft Office Suite (Word, Excel, Outlook, PowerPoint).

Ability to learn and navigate case management and referral tracking software.

Familiarity with telehealth platforms and secure communication tools.
