Job Summary

Odyssey House Louisiana, Inc. is seeking a full-time Lead Case Manager. The Lead Case Manager will supervise the day-to-day work of the Case Management team along with maintaining a caseload. The Lead will support onboarding and training of all program staff. The Lead will be expected to act as liaison for the team and provide both internal and external support for resources and referrals.

 

Responsibilities and Duties

It is the duty and responsibility of the Lead Case Manager to:

·        act as a role model for staff and guests

·        promote agency culture and values

·        ensure staff adhere to agency-wide and program specific policies and procedures;

·        ensure that guests receive quality assistance as evidence by case work documented, and implement and document corrective actions as needed;

·        provide support to case management team with an assigned caseload of approximately thirty or more individual(s); Lead may be expected to manage a caseload if census dictates or if complex cases are present.

·        prepare and submit reports to Project Manager in a timely manner;

·        verify the results of the assistance provided in order to close the case, as per the approval of the Program Manager;

·        review written progress notes of work done by the case management team for quality purposes;

·        provide crisis management when indicated by individual(s) needed;

·        make necessary referrals to fill potential gaps in assistance;

·        review data entry of individual’s information into the appropriate agency’s database(s) in a timely matter and update as needed;

·        attend all required meetings and trainings;

·        recommend process improvements and trainings as needed;

·        prepare work schedules and determine caseloads

·        completes all Incident Reports prior to the end of scheduled shift as required by Incident Policy and Procedure;

·        maintain individual licensing requirements;

·        oversee daily individual activities for individual(s) in the LBS as needed;

·        perform other duties as assigned.

 

Qualifications and Skills

Required

·        Bachelor’s Degree

·        At least one year supervisory experience

·        Excellent communication and organizational skills

·        Ability to work with homeless individuals of diverse backgrounds and disabilities including physical, mental, substance abuse, immunodeficiencies, etc

·        Database management experience

·        Ability to navigate twelve passenger van

 

Preferred

·        Master’s Degree

·        Experience with Target population
