Position Overview:

The DCPS will ensure the delivery of quality patient care through professional assessment, intervention, evaluation and supervision of the patient care team and plan of care. The DCPS provides supervision to all nurse case managers, on-call nursing staff, admissions nurse and hospice aides under the direction of the agency Administrator and Medical Director. The DCPS ensures quality of staff and functions of the company through management of policies and procedures, state and federal regulations and budgeting. Essential Duties & Responsibilities:
Maintains compliance with federal and state regulations as appropriate.
Promotes hospice referrals in the community.
Supervise the planning, coordination, and safe delivery of patient/family care.
Assigns each patient/family to appropriate staff members.
Plan and/or conduct IDG conferences. Present cases as indicated and participate in the discussion, planning, re-evaluation, and updating patient Plan of Care.
Conducts supervisory visits with patients and staff as appropriate.
Audits medical records, checking for accuracy, documentation, and appropriate patient care.
Evaluate the performance of the staff.
Monitors continuous case situations to assure that at least the nursing staff provides 51% of hands-on patient care.
Participates in community programs as requested, to promote professional growth and understanding of hospice care.
Provides the community with accurate information.
Initiates emergency procedures when necessary.
Communicates information effectively to appropriate personnel to facilitate continuity of care.
Accepts accountability for own practice.
Maintains competency by participating in in-services and continuing education programs.
Documents all patient-related activities in a timely manner.
Reacts to change productively and performs other job-related tasks and duties as assigned.
Assesses a patient's condition and family needs through the collection of health data and the recognition of psychological, social, and spiritual factors that may influence the patient/family plan of care. Re-evaluates patient/family needs as required.
Provides integrated team process leadership for leadership team members in representing areas of responsibility.
Skillfully presents information to the Board of Directors and maintains positive relationships with community members and referral sources.
Partners with the CEO to represent Unified Care Hospice with external constituency groups, including community, government, and private organizations.
Build an effective and caring staff organization.
Develop short and long term goals/objectives and strategic plans and approaches to accomplish them.
Responsible for the measurement and effectiveness of all internal and external processes.
Provides timely, accurate and complete reports on the operating condition of the Hospice.
Ensures that all program activities in areas supervised operate consistently and ethically within the mission and values of the organization.
Reviews, negotiates and approves all contracts in conjunction with the CEO.
Oversee Clinical Operations
Fosters and monitors collaboration with the senior management team.
Attracts, recruits and retains required members of the team; provide mentoring to meet the leadership needs of this growing organization.
Leads and coordinates the research, planning, design, implementation and evaluation of significant projects and organizational change efforts.

Demonstrates understanding of HIPAA Privacy Standards and related Agency policies.
Demonstrates understanding and compliance with agency policies, including but not limited to, documentation, continuing education, attendance/work schedules and required HR documentation, State and Federal policies and procedures, laws and regulations.
Attends agency functions as needed outside of office hours.
