Unified Care Hospice is seeking a compassionate and dedicated Certified Nursing Assistant (CNA) to join our mission-driven team. Key Responsibilities:
Assist patients with activities of daily living (ADLs), including bathing, grooming, dressing, and toileting.
Provide companionship and emotional support to patients and their families.
Observe and report changes in patients’ physical, emotional, and psychosocial well-being to the nursing team.
Maintain accurate and timely documentation of patient care activities in electronic medical records (EMR).
Work collaboratively with nurses, social workers, chaplains, and other healthcare professionals to develop and implement personalized care plans.
Help ensure a safe, clean, and comfortable environment for patients.
Uphold the highest standards of compassionate, patient-centered care throughout every interaction.
Qualifications:
Active Certified Nursing Assistant (CNA) certification in the state of Nevada (required).
Hospice or home health experience preferred, but not required—we provide training and mentorship.
A deep sense of empathy and commitment to providing comfort to patients and families.
Strong communication and interpersonal skills.
Ability to collaborate effectively within an interdisciplinary team.
Reliable transportation for patient visits (mileage reimbursement provided).
Comfortable with travel and a flexible schedule.
COVID-19 vaccination required.
Schedule & Availability:
Day, Evening, Night, or Overnight shifts available.
Monday–Friday with weekends as needed.
Additional hours may be available on holidays or as needed.
