Title:                            Integration Support Worker (Grid 21) -
Location:                     Abbotsford Community Hub
Position Status:           Full-time (Temporary) – End on March 31st, 2025, 37.5 hours per week.
Reports to:                  Program Manager
Compensation:            $27.92-$29.60 

Job Summary:

 As a member of the Phoenix Society, Clinical and Outreach Teams based out of the Abbotsford Community Hub, the Integration Support Worker provides a variety of services to those living with viral hepatis and/or HIV currently incarcerated, or recently released into community with connections to information about blood borne diseases, community referrals, treatment plans, and case planning. This role includes, facilitation, transportation supports, statistic collection, education, outreach and evaluating service delivery. Please note that the position is temporary and will end on March 31st, 2025.

  

Responsibilities: 

Client Services

·       Liaise with other support workers, medical providers and agencies, providing one-on-one support to clients who are currently incarcerated and support with transitional plans to continue their medical treatment once released
·       Outreach into Federal Institutions, providing discharge planning for inmates who are diagnosed with HCV or at risk of infection
·       In partnership with the BCCDC, will deliver educational programming both in correctional environments and in community
·       Providing accessible and stigma-free Point-of-Care testing for HCV.
·       Perform new client intakes as assigned
·       Create and maintain client case planning documentation
·       Create, include and liaison with the peer support committee
·       Distribute Harm Reduction supplies and resource materials
·       Provide transportation to high-risk clientele for medical care and appointments using personal vehicle, or company vehicle, if available
·       Maintain accurate case notes and statistics regarding program activities, number of participants, contact and referral information, etc.
·       Maintain confidentiality and professionalism at all times

 

Operations

·       Identify areas for improvement and increased efficiency in program operations, services offered, reporting and reports to supervisory positions
·       Follow and implement universal precautions and infection control adhering to all health and safety protocols
·       Ensure safe handling of sharps containers and other equipment
·       Ensure that program regulations, policies and procedures are followed
·       Maintain up-to-date knowledge of internal fire safety policies and procedures, as well as other emergency procedures
·       Attend professional development trainings as directed
·       Organize and assemble monthly administrative reports
·       Maintain agency standards of work quality

 Administration
·       Provide administrative support, including but not limited to: scheduling, typing, photocopying, attendance at meetings; documentation or data entry; creation of documents; or other duties as assigned
·       Provide reports to the Program Manager as requested/required

 Community Relations

·       Support organization of community events and attends events providing the public with information and awareness resources
·       Facilitation of groups and informational events
·       Participate in liaison activities with stakeholders and the broader community in a professional manner
·       Maintain a thorough knowledge of social, economic, recreational, and educational resources in the community. Liaise with other outreach workers, medical providers, and Fraser Health
·       Foster teamwork within Phoenix Society and with service stakeholders
·       Provide the public with information and awareness resources by facilitating groups and participating in informational events

 

Qualifications: 

·       Post-secondary diploma (e.g., 2 to 3 years) in social/human sciences, addictions or other equivalent education in the medical field such as LPN or Health Care Aide experience
·       Note: Preference will be given to those who have post-secondary education and training relevant to the social service and medical field. Alternately, those currently pursuing a certification in social work, counseling, criminal justice or a related field, or the equivalent combination of education and experience will be considered for this position.
·       Minimum 2 to 3 years’ experience supporting vulnerable populations, involved in the criminal justice system
·       Minimum 2 to 3 years’ experience working with vulnerable populations and community resources, preferably with clients living with HCV/HIV
·       Understanding of HIV and Viral Hepatitis transmission and treatment is an asset
·       Demonstrated understanding of the criminal justice system
·       Satisfactory completion of a criminal records check with vulnerable sector screening is required; ability to obtain Federal Clearance
·       Satisfactory completion of Employee Immunization Record form is required
·       First Aid 1 and Current CPR certificate is required
·       Valid driver’s license accompanied by an approved driver’s abstract is required

  • Knowledge of Windows, Microsoft Word, Excel, and database management systems
  • Knowledge of case management processes and procedures; trained and experienced in professional record keeping, report writing and statistics

Salary for this role is between $27.92-$29.60 per hour.
