Aberdeen Place Hospice is seeking an experienced Hospice RN Case Manager! Responsibilities:

The Hospice RN Case Manager will be responsible for a manageable caseload of Hospice patients providing development and implementation of each patient’s plan of care. The Hospice RN Case Manager continuously plans, implements, and evaluates patient care incorporating patient and family into the process, provides direct patient care, and functions as a patient advocate. Responsible for participating in the development of an interdisciplinary plan of care and ensuring the overall coordination and management of hospice services for patients and their families

  • Responsible for the day-to-day management of the patient's plan of care while ensuring palliation of symptoms related to the terminal illness.
  • Maintains a nursing service philosophy and related rules, standards and practices; establishes care plans, including assessments, plans of treatment, objectives and goals, evaluations, and discharge planning, maintains current medication profiles, and timely reorder of hospice appropriate medications.
  • Ensures compliance with regulations and rules related to care, certification, and licensing; professionally represents Aberdeen Place Hospice to residents, residents’ families, outside agencies, care providers, and the public.
  • Maintains liaison role with all levels of administration, physicians, and outside organizations.
  • Performs patient admissions as needed.

Ideal Candidate:

  • 2+ years as a Hospice RN Case Manager
  • Experience working with Senior Living communities
  • Excellent people and relationship building skills
  • Ability to increase patient census by providing excellent care, responsive service, and relationships

Qualifications:

  • Bachelor of Science degree in Nursing (BSN) desired
  • Current GA license as a Registered Nurse. Current CPR required.
  • Knowledge and understanding of both GA State and Federal Rules & Regulations governing Hospice
  • Excellent assessment skills, communication skills and ability to establish a therapeutic interpersonal relationship with patients and caregivers
  • Experience using Consolo software a plus
  • Excellent interpersonal, organizational and communication skills
