Location: Ohio – Hybrid or Office-Based

Pay: $16.50/hour

Position Summary

We are seeking a skilled and organized Home Health Referral Coordinator to manage and facilitate home health referrals for patients in need of medical services. In this role, you will be the primary point of contact for initiating, processing, and coordinating home health services, ensuring timely and accurate service delivery to patients in need of care.

Key Responsibilities
  • Process and track home health referrals from physicians, discharge planners, and other healthcare providers

  • Communicate with home health agencies to arrange patient evaluations and services

  • Verify insurance coverage, obtain authorizations, and ensure compliance with Medicare/Medicaid and insurance requirements for home health services

  • Maintain accurate documentation of all referral details and follow-up actions in the EMR and tracking systems

  • Coordinate communication between healthcare providers, patients, and families to ensure a smooth transition to home health care

  • Provide support and guidance to patients and families regarding home health services, eligibility, and scheduling

  • Assist with scheduling appointments and services in a timely manner to meet patient needs

  • Participate in internal team meetings to ensure efficient service delivery and patient satisfaction

Qualifications
  • High school diploma or GED required; Associate’s or Bachelor’s degree in health sciences or a related field preferred

  • Experience in home health coordination, care management, or medical office administration is preferred

  • Knowledge of home health service delivery, insurance verification, and referral processes

  • Strong organizational skills and the ability to manage multiple tasks simultaneously

  • Excellent communication skills, both written and verbal

  • Proficiency with EMR systems and Microsoft Office Suite

  • Ability to work independently as well as part of a team

  • Compassionate and patient-focused approach to coordinating care

Preferred Qualifications
  • Experience with Medicare/Medicaid or private insurance

  • Bilingual in Spanish or another language (a plus)
