Home Health Admission RN

Location: Rochelle, IL and the surrounding area

Schedule: Weekdays Monday-Friday 8a-5p with flexibility

Case Specific: The Home Health Admission RN will complete Start of Care (SOC) visits, perform 30-day supervisory visits for LPNs, handle recertifications post-hospitalization or after 30 days of care, and provide skilled nursing visits for PICC line management. Must be detail-oriented and efficient with documentation.

Job Summary:
The Home Health Admission RN is responsible for initiating patient care through Start of Care (SOC) admissions, conducting supervisory visits for LPNs in their assigned region every 30 days, and completing recertifications following 30 days of care or post-hospitalization. This role also includes providing skilled nursing visits for PICC line management and ensuring all documentation is completed accurately, thoroughly, and in a timely manner. The ideal candidate demonstrates strong clinical judgment, organizational skills, and the ability to manage a high volume of documentation with precision and efficiency.

• Active RN license in Illinois
• Oasis experience
• Strong clinical skills with home health experience preferred
• Experience with wound care, IVs, catheters, feeding tubes, labs, and patient triage
• Compassionate, patient, happy, and outgoing personality
• Self-starter with excellent communication and interpersonal skills
• Reliable transportation to visit patients
