SUMMARY: The CC serves as the onsite HIV Coordinator while providing Care Management services for out HIV/PrEP patients. The CC maintains open, excellent communication with Providers to safeguard quality care. The CC reports to the Medical Director.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Is knowledgeable of and applies the Mission and Values of ARMS.
  • Serves as primary support staff to assigned Physician, Nurse Practitioner or Physician Assistant.
  • Responsible for the Health System Navigation of HIV/PrEp patients desiring treatment.
  • Coordinate the identification of patients who were screened for HCV;
  • Collection of demographic and clinical information, data regarding HCV treatment outcomes as well as Patient Reported Outcomes for diagnosed patients.
  • Assist with education training efforts, facilitate collection of data, enter patient data in the secured reporting electronic database, and ensure data quality at the different sites.
  • Assist ARMS providers, local health officials, and emergency department staff by supporting linkage, retention, and treatment services to newly diagnosed persons.
  • Provides day-to-day support and clinical guidance to medical assistant staff in order to maintain an even flow of patients in the clinical area.
  • Assists providers, Nurse Practitioners, Physician Assistant, and Care Managers in the delivery of quality health care to patients.
  • Assists in the VFC orientation of new employees and student experiences as well as yearly training of all medical staff.
  • Supports policies, procedures, and supervisory decisions within the Health Center.
  • Participates in continuing education programs as requested.
  • Uses established channels of communication and proper lines of authority for problem solving and communication.
  • Assists in documentation of incidents and untoward events and participates in safety programs and disaster/fire drills.
  • Maintains an acceptable attendance record; provides proper notification for absences or tardiness; documents overtime including justification.
  • Is knowledgeable of patients’ rights and responsibilities and accepts responsibility for the assurance of the same. Maintain and assures confidentiality in accordance with HIPAA Guidelines.
  • Assist with patient Triage.
  • Assist with Medical Chart audits.
  • Assist in the care coordination of the HIV Screening Grant.
  • Performs other duties as assigned.

QUALIFICATION REQUIREMENTS:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions.

Must be a LPN that has successfully completed an accredited LPN Program

Must have 2-5 years of clinical experience

Supervisory experience is preferred

Experience with patient referrals and tracking

Must be willing to travel

OTHER SKILLS AND ABILITIES:

Must possess skills in operating a personal computer, office fax machines and copiers

Excellent organizational and leadership skills with the ability to pay attention to details

Exhibits professional behavior at all times

Effective and respectful communication skills required both written and verbal

Experience and desire to work in a holistic and culturally diverse health care environment serving the under-insured and uninsured.'
