Hourly Per Diem Rate: $22 - $25

Qualifications:

• At least 1 year experience as an HHA or C N A.
• Reading and writing proficiency in English required
• Mandatory criminal background check
• Preferably CPR certification
• Driver record check, reliable transportation, and liability insurance are mandatory.
• Qualifications meeting one of the following criteria:
  • Completion of a home health aide training program and certification
  • Completion of a nurse aide training program meeting state requirements

Responsibilities:

• HHA tasks assigned and supervised by RN.
• Duties include ADLs, personal hygiene, meal prep, ambulation.
• Assist with treatments and therapy.
• Promote independence, mental alertness, emotional well-being.
• Establish trust, report changes in patient status promptly.
• Attend required in-services, confirm visit schedules weekly.
• Other duties as assigned.
