Purpose of Position

We are proud to be a nurse-led, multidisciplinary clinic located within the Covenant House crisis shelter, offering essential primary care to young people who are experiencing or at risk of homelessness and/or human trafficking. Our services address a wide range of health concerns, including physical, sexual, and mental health, substance use, prenatal care, crisis management, as well as the health needs of 2SLGBTQIA+ individuals and refugees. With over 5,000 visits annually, our dedicated team collaborates closely with family doctors, psychiatrists, and an adolescent medicine specialist to provide comprehensive care five days a week. Guided by our mission of unconditional love and absolute respect, we strive to make a meaningful impact in the lives of those we serve every day.
We are seeking a dynamic and experienced manager to lead the operations of our healthcare clinic, ensuring that service delivery aligns with the practice regulations set forth by the College of Nurses of Ontario (CNO). Reporting to the Director of Clinical and Health Services, the manager will work closely with front-line staff to provide holistic, youth-centered care, while also playing a key leadership role within the organization. Additionally, the manager will cultivate and maintain strategic partnerships with external healthcare partners to enhance service delivery and foster collaborative relationships.
Responsibilities:
Program and Management
• Responsible for ensuring that services delivered to youth align with Covenant House principles and policies, while adhering to guiding service principles, including a trauma-informed, harm-reduction, and strengths-based framework
• Ensures that nursing procedures, delegated controlled acts, medical directives and the implementation of RN Prescribing are in accordance with the CNO Standards of Nursing Practice
• Oversee the clinical supervision of healthcare staff, including conducting coaching sessions, performance evaluations, and auditing clinical documentation
• Responsible for ensuring health care programs remain responsive to the needs of youth and developing programs that respond to those needs
• Participates in the strategic planning process to identify objectives for this program area
• Plans, facilitates, and coordinates with our healthcare custodians, Inner City Health Associates, and other community partners
• Collaborating with healthcare providers to plan and implement medical directives
• Organizes relevant training for registered nurses and program staff
• Coordinates and approves departmental and health related policies and procedures
• Negotiates with community agencies and practitioners to maintain high-quality services while identifying gaps in care, developing models of care, and securing potential funding to address the needs of our target population
• Participates in the agency’s Program Management Team to address agency issues
• Potential for on-call during the implementation of the weekend pilot program
• Ensures alignment with Covenant House Toronto’s strategic plan
Service Delivery:
• Performs comprehensive nursing assessments, demonstrates clinical competency and ability to work effectively in a multidisciplinary team to deliver holistic care
• Provides and directs infection control cases, including Public Health Department follow up
• Works with our Harm Reduction staff to ensure proper tools, kits and other items are obtained, are in good supply and are effective
Human Resources Management:
• Ensure that all human resource policies are adhered to consistently throughout the department. Provides leadership, sets expectations, and promotes effective working relationships within the staff team
• Demonstrates an awareness of organizational health and safety policies and understands their specific responsibilities within the health and safety program
• Support in the on-boarding of new clinical staff, ensuring a smooth transition and integration into the team
• Integrates injury and illness prevention into all job activities
• Directly supervises, appraises and evaluates the nursing team and healthcare coordinator
• Makes recommendations to the Director of Program on hiring, appraising and terminating of all direct report staff
• Identifies training needs of service staff
• Plans and participates in clinical presentations to medical and nursing students, and clinicians.
• Participates in the delivery of information on a variety of health and wellness related subjects to staff
Administration:
• Prepares, implements, and analyses annual budget/service plan for department, including fiscally and professionally responsible resource allocation.
• Approves all expenditure requests and ensures department expenses are consistent with budgetary allotments.
• Maintains statistical collection system and analysis of monthly data for department.
• Participates in relevant Community Groups (e.g. Street Nurses’ Group, Community Clinics).
Qualifications:
• Degree from an accredited College or University (BScN, RN extended class)
• Registered and in good standing with the College of Nurses of Ontario (CNO)
• Professional liability protection (PLP) required
• 3-5 years’ of direct nursing care experience working with adolescents in community health, primary care and/or mental health – manager experience is preferred
• Certification in RN Prescribing is an asset
• Demonstrated competency in performing comprehensive and holistic nursing care
• Demonstrated ability to work as an independent clinician who is a contributing member of a multidisciplinary team
• Excellent written and verbal communication skills, with demonstrated experience in supervision and leadership
• Strong computer literacy and proficiency in various software and tools, experience with OSCAR is an asset
