Foster Care Crisis Case Manager
Reports To: Director of Foster Care
FLSA Status: Exempt
Job Summary: This position will be the primary case manager for all youth but emphasis on treatment foster youth in care. In certain cases, based on the special needs of the children served, position may be gender specific. Responsible for aligning individual and family services for children in the foster care program. Crisis support staff to promote our clients' functioning. Performs crisis support aid and assists in the design of certain protocols and then works with foster families to execute the strategies identified therein, as needed.
To ensure success as crisis support staff, Crisis Case Manager should demonstrate responsive flexibility and steadfast calmness. Ultimately, superb crisis and calming support staff will remain cognizant of often imperceivable environmental irritants, which they will calm or adapt to as is fitting. Must be a self-starter capable of working independently; Must be dependable and work effectively with minimum supervision.
Duties:
Provides skills training and coaching to children and families in accordance with Three Rivers
Youth prescribed curriculum and trainings.
Provides consultation, instruction and direction to other staff, clients, and referral agencies.
Interviews, clients to assess functioning to develop client’s initial needs and individual crisis plan.
Completes all CANS assessments on youth in the program.
Identifies, develops, supports, and coordinates recommendations for clients.
Assists in the development of the initial and subsequent service plans that meets client’s needs and complements services provided by other treatment providers.
Advocates, refers, and directs support services to clients.
Completes written documentation of ongoing services of children and progress through discharge.
Assists in the development of discharge and aftercare plans.
Provide crisis intervention and on-call support.
Maintain child and/or family records in Arrow’s electronic client information system utilizing assigned Arrow technology resources. These resources may include, but are not limited to
desktop computers, laptop computers, touchscreen devices, and scanners.
Utilize (personal) cell phone mobile data hotspot for in-field access to Arrow’s client information system in order to complete documentation in accordance with job workflow.
Review client files on a quarterly basis to access service plan implementation, progress towards desired outcomes, and future appropriateness of service plan.
Developing trusting connections with clients that foreground respect.
Co-developing custom made, evidence-based intervention strategies alongside vested parties.
Tailoring these custom-made regimens if required.
Creating structured and emotionally supportive environments in accordance with treatment plans.
Facilitating the adaptive modification of unsuitable behaviors.
Eliminating or cushioning the effects of disruptive sensory stimuli.
Collaborating with our clients' families, instructors, and consulting Clinicians, where needed.
Assists in the development of individualized progress reports and service plans.
Promoting individual and familial self-sufficiency.
Preserving our clients' safety and independence.
May be required to drive TRY vehicles, be insurable with our auto insurance provider, and complete training.
Perform all duties within contract, state and federal regulations, TRY policy, State Licensing
Board Standards, and COA standards.
Have working knowledge of the social services, child welfare, kinship, or foster care systems, 3680, 3700 regulations and foster care best practices guidelines.
Maintains confidentiality of all client-based information as required by law.
Maintains records and/or data. Completes monthly, quarterly, and annual reports in a timely manner.
Serves as case manager to assigned clients. Maintains family records according to program standards and expectations. Ensures client records are complete are regarding regulatory requirements and that all documentations are present and clear.
On an as needed basis, may be required to attend court hearings.
OTHER RESPONSIBILITIES:
Maintains 25 training hours per year.
Maintains open communication with supervisor and other staff as it relates to the necessary flow of the job and programs.
Meet with supervisor on a weekly basis to review performance and/or issues.
To be a flexible member of the agency's team to accomplish the overall agency goals.
Possesses and utilizes the ability to relate to problems and special needs of clients.
Must possess and utilize excellent verbal and written communication skills and general computer skills, including Windows, WORD, EXCEL, email, Internet, and other systems.

WORKING CONDITIONS:
While performing the duties of this job, the employee is regularly required to talk and hear. The employee is frequently required to stand, sit, and walk indoors and outdoors for long periods of time weather permitting; use hands; reach with hands and arms. The employee must be capable of lifting or pulling 175 lbs. Specific vision abilities required by this job include close vision and the ability to adjust and focus. Proper diction and command of the English language required.
QUALIFICATIONS:
A Master Level Professional is necessary for this position. A Master’s Degree from an accredited college or university, preferably in the social services as well as two years’ experience in therapeutic work with families or adolescents is preferred but not required. Basic knowledge and understanding of child development, trauma informed care, family systems and psychodynamic theory is necessary. Verbal and written communication skills and individual and group counseling is required. Individual must have a valid PA driver’s license and reliable transportation.
Act 33/34 Child Abuse Clearance and Pennsylvania Criminal History Clearance, and FBI Clearances are required.
