Family & Natural Supports Coordinator - Foundry Kelowna

Kelowna, BC Full-time $30.54-$35.12/hour

WHO IS A FAMILY & NATURAL SUPPORTS COORDINATOR?

The Family & Natural Supports (FNS) Coordinator plays a critical role in fostering connections and support networks for youth at risk of homelessness and dislocation. By emphasizing the core principles of the Family and Natural Support Program, the Coordinator will empower youth to build and strengthen their natural support systems, ensuring a holistic approach to their physical, social, and emotional needs.

This is a full-time position offering 37.5 hours per week, with a typical work schedule of Monday-Friday, 9am-5pm with a monthly shift variant on Thursdays from 11am-7pm.

WHAT DOES A FAMILY & NATURAL SUPPORTS COORDINATOR DO?

  • Assist youth in taking ownership of their care and decision-making to positively influence their life trajectory
  • Identify and connect participants with relevant social, economic, recreational, and educational resources that can provide care and assistance during challenging times
  • Conduct assessments to evaluate youth’s housing history and identify supportive relationships
  • Engage in collaborative case planning with participants and their supports to facilitate success
  • Facilitate mediation and reconciliation for youth and their support networks to strengthen familial relationships
  • Deliver brief supportive counseling and make referrals to appropriate mental health services when needed
  • Maintain accurate records including case files and progress reports
  • Address participant feedback, inquiries, and complaints promptly
  • Provide training in systems navigation, including access to community resources tailored to individual needs
  • Offer education on topics such as Harm Reduction and Trauma-Informed Care
  • Monitor participants for signs of potential emergencies and develop strategies to address challenges
  • Document and report any medical, behavioral, or legal issues as required
  • Performs other related duties as assigned

 STRENGTHS & EXPERIENCE:

  • Degree or diploma in Social Work, Human Services or related field required
  • Minimum 2 years’ relevant experience working with youth and families
  • Ability to be regularly exposed to individuals who are struggling with and/or recovering from trauma, drug/alcohol use, homelessness, mental health challenges and/or mental illness
  • Reliable vehicle and the ability to meet/transport clients within the community on a frequent basis
  • Valid Class 5 driver’s license
  • Criminal Record Check clearance in the Vulnerable Sector
  • Certificates in First Aid, CPR and Naloxone considered an asset
  • Ability to maintain consistent professional boundaries
  • Proficient Microsoft Outlook, Word and Excel
  • Strong theoretical foundation in social systems, human behavior, and support strategies
  • Proficient in record-keeping and administrative tasks, including case management software
  • Strong attention to detail and the ability to prioritize tasks and manage time effectively in a dynamic environment
  • Strong analytical and problem-solving skills to assess participant needs and develop appropriate interventions
  • Conflict resolution, mediation and crisis intervention skills
