Hands of Peace Home Care is a non-medical home care agency servicing adults with intellectual / developmental disabilities in the Charlotte Mecklenburg area. HPHC is looking for passionate and energetic team-players, experienced in Competitive Integrated Employment (CIE) and 1915i Supported Employment Services.

The successful candidate will have the aforementioned experience and be skilled at connecting with people and building relationships, while demonstrating excellent client service, reliability, and advocacy skills.


POSITION QUALIFICATIONS:
  • Excellent communication skills, flexible and able to interact positively with staff and clients.
  • Passionate, must present a positive and professional business image
  • Hold valid driver's license and automobile insurance
  • Demonstrated knowledge of Competitive Integrated Employment and 1915(i) Supported Employment Services
  • Strong documentation skills to reflect service preparation for employment
  • Experience working with individuals with I/DD and/or Autism
  • A negative TB screening is required upon employment
  • Experience working with teens and young adults, preferred
  • First Aid/CPR certification, preferred
  • Must be able to successfully pass a background check

KEY RESPONSIBILITIES:
  • Job Placement and Coaching: Facilitate job placement, coaching, and customized employment for individuals with I/DD.
  • Transportation Coordination: Arrange transportation services to various service delivery locations and job sites. Must be willing to transport client as needed.
  • Evidence-Based Support: Utilize evidence-based models endorsed by the Office of Disability Employment Policy (ODEP) to support individuals with I/DD and traumatic brain injury (TBI).
  • Recruitment: Source and recruit youth with disabilities from the local school system and facilitate enrollment in program services.     
  • Program Development and Leadership: Spearhead the development and expansion of the employment support program.

SUMMARY OF JOB FUNCTIONS:

  • Career and Educational Counseling: Assist clients with career exploration, discovery, and creating Career Development Plans (CDP).
  • Job Readiness and Development: Provide comprehensive training in job readiness skills, active job searching, job shadowing, resume preparation, and interview skills.
  • Job Development Activities: Identify and secure suitable employment opportunities, assisting clients in self-employment ventures, and providing technical assistance to potential employers regarding ADA accommodations.
  • Benefit and Support Services: Coordinate benefits counseling, assist with documentation for benefit management, and develop long-term work travel and transportation plans.
  • Technical Assistance: Conduct detailed reviews of client’s employment service delivery record, hold progress meetings, provide on-site job support and advocacy, and develop targeted action plans to overcome employment barriers.
  • Self-Employment Assistance: Support clients in identifying viable business opportunities, creating robust business plans, and obtaining essential resources for successful self-employment.
  • Outreach and Community Engagement: Establish and maintain strong relationships with employers, community organizations, and educational institutions to promote program services and advocate for clients’ employment opportunities.
  • Compliance Management: Ensure adherence to all applicable policies, procedures, standards, laws, rules, and regulations, maintaining compliance integrity across all aspects of service delivery.
  • Oversee Employment Support Program: Drive the development and launch of our employment support program, including the creation of program materials, documentation, and training resources. Lead training initiatives to ensure high-quality and effective service delivery.
