Director of Social Work

Long Island Care Center is seeking to hire a Director of Social Work with experience working in a Skilled Nursing environment to join our dynamic team of professionals.

Nursing home experience preferred

Responsibilities of the Social Worker include, but are not limited to, the following:

• Completing MDS assessments.
• Facilitate care plan meetings & discharge planning.
• Implement History and Assessment, Advanced Directives and End of Life Planning.
• Audit psychology paperwork, hospital discharges, room changes, PASRR, and Advanced Directives.
• Completing all required documentation in the medical record (EMR)
• Coordination of care and services including ongoing monitoring of an appropriate and effective person-centered care plan; Strong Clinical, Communication and Organizational Skills
• Demonstrates judgment of safety and confidentiality issues
• Strong verbal and written communication skills
• Strong customer service focus required
• Knowledge of the community resources
• Working Knowledge of Sigmacare

* Good working knowledge of long-term care regulations including documentation requirements

EDUCATION:

Bachelor’s degree in social work or related field.  Experience working as a social worker in a Skilled Nursing Facility.

Salary: $90,000 per year
