Liberty Health Care Center is hiring a Director of Nursing (DON) who is proactive, accountable, and relentlessly driven for excellence. You will serve as the executive clinical leader for the building, overseeing quality outcomes, regulatory performance, team capability, and the day-to-day delivery of resident-centered care.
Executive Clinical Leadership
Set a clear clinical vision with measurable goals; run a disciplined cadence of daily huddles, IDT rounds, and KPI reviews.
Ensure the right staffing/skill mix each shift; deploy evidence-based practices and close gaps quickly.
Regulatory Mastery & Risk
Maintain continuous survey readiness (CMS RoPs, State regs, Life Safety); lead tracers, audits, and POCs.
Oversee infection prevention, antibiotic stewardship, incident review/RCAs, and policy adherence.
Quality, Outcomes & Performance Improvement (QAPI)
Own results for falls, wounds, infections, weight loss, pain, rehospitalizations, and QMs/Five-Star.
Lead QAPI with data-driven PIPs, dashboards, and tight follow-through to sustain improvements.
People Leadership & Talent Development
Build a high-performing nursing team: recruit, onboard, set expectations, coach, and hold accountable.
Drive competency validation, preceptorships, clinical ladders, and succession planning for RNs/LVNs/CNAs.
Foster a Just Culture with timely feedback, recognition, and performance actions when needed.
Clinical Documentation & Reimbursement Alignment
Partner with MDS/RAI, Therapy, and Medical Staff to support accurate assessments, ICD-10 coding, and documentation aligned to resident needs.
Ensure eMAR/EMR integrity, MAR/TAR accuracy, and timely orders/notifications.
Operations & Fiscal Stewardship
Collaborate with the Administrator on labor (HPPD), agency reduction, supplies/PPD, and schedule integrity.
Use weekly flash/monthly financials to manage variances; align clinical programs with operational goals.
Resident, Family & Community Relations
Be a visible leader—round with purpose, resolve concerns early, and communicate with empathy and clarity.
Strengthen collaboration across IDT, providers, pharmacy, and therapy for seamless, resident-first care.
Stable staffing plan and schedule integrity with reduced agency reliance.
Active QAPI with 2–3 targeted PIPs showing early movement in priority metrics.
Clean documentation audits; zero tolerance for MAR/TAR defects.
Documented survey readiness plan with tracer findings addressed.
Clear coaching plans for leaders and preceptors; improved team engagement signals.
Current, unencumbered RN license (Texas) required.
5+ years progressive nursing leadership in SNF/LTC (ADON/DON level preferred).
Demonstrated success in improving QMs/Five-Star, survey performance, and reducing readmissions.
Deep working knowledge of CMS regulations, infection prevention, QAPI, and clinical standards of practice.
Proven people leader: builds teams, develops talent, and holds standards with fairness and urgency.
Strong operational acumen: staffing models, budgeting, KPI management, and EMR/eMAR proficiency (e.g., PointClickCare).
Able to participate in on-call support, with occasional evenings/weekends/holidays as needed.
