JOB DESCRIPTION: Community Liaison

REPORT TO: TEAMCARE COORDINATOR
JOB SUMMARY:
Serves in a consultant role, acting as an advocate and marketer of our services to community members, out-patient clinics, physician offices, churches, schools, nursing homes, and and other clinical settings. § Maintain a thorough knowledge of our community’s admission requirements, services and programs. Interact with the general public, potential residents, residents, staff, and others to explain these requirements.
§ Support and promote the philosophy, goals and objectives of the sales and marketing programs and activities.
§ Assist assigned prospect referrals with the process of enrollment for Home and Community Based Services
§ Demonstrate knowledge of clinical agency standards and criteria of agency admission.
§ Communicates with Intake RN Department and/or Clinical Administrator or Acting Clinical Admin to assess each prospective resident to determine the appropriateness for admission.
QUALIFICATIONS:
Education/Job Requirements
- Associates or Bachelor's Degree in Human Services, Social Worker, Nursing and/or 3 plus years of experience in related field of Home and Community Based Services
- Must demonstrate satisfactory completions of any stated mandated training.
- Proficient in Computer Skills (E-mail/ Electronic Medical Records Documentation)
- Applicant must be bondable and meet or exceed minimum qualifications for each of the following background checks:
- Criminal Background Investigation
- Motor Vehicle Driving Record
- Professional and Personal Reference Checks
- Give permission to submit to random drug and alcohol testing.
- Must have reliable transportation and fulfill assignments with reliability and punctuality.
- Must have a valid driver's license and automobile insurance (If Applicable)
Language Skills
Bilingual with emphasis with understanding medical terminology. Must be able to follow verbal and written instructions and document services provided. Must be genuinely concerned about helping people and have high moral standards of honesty and integrity
Essential Duties/Responsibilities:
- Ensure that public information (policy manuals, brochures, information packets, etc.,) describing the services provided is accurate
- Assist all prospect referrals with the process of enrollment for Home and Community Based Services
- Provide daily tasks logs/call logs when applicable
- Attend (1)- 15 minute weekly virtual meeting with Intake RN Department/Clinical Administrator (virtual vs. in-person)
- Develops strong relationships with key referral hospital personnel and identify new opportunities for partnering and growth.
- Promotes going outside of the facility to make contact with any healthcare providers who may benefit from your facility’s services. These contacts include nurses, physicians, case managers and social workers.
Working Environment: Patient’s home setting, community outreach (outdoors and indoors). Contact with blood or other body fluids may pose a risk for exposure to blood borne pathogens and infectious diseases.
Position Physical Demands: The work requires medium to heavy physical exertion and lift up to 100 lbs on a regular and reoccurring basis, such as driving, assisting the participants in activities when applicable.You are regularly required to sit, walk, talk, hear and occasionally required to reach and lift. IBH requires all employees prior to any offer of employment being extended; all employees must successfully pass a state mandatory criminal background check. IBH is prohibiting from hiring and or retaining any individual(s) with a prohibited conviction or Department of Aging ineligibility determination.
As required under PA Code § 611.52. (e) Prohibition. The home care agency or home care registry may not hire, roster or retain an individual if the State Police criminal history record reveals a prohibited conviction listed in 6 Pa. Code § 15.143 (relating to facility responsibilities), or if the Department of Aging letter of determination states that the individual is not eligible for hire or roster.
