This position supports the work of the Y, a leading nonprofit, charitable organization committed to strengthening community through youth development, healthy living and social responsibility. As a Community Health Coach, you will support the Y in one or multiple of the Evidence Based Health Intervention (EBHI) programs Rock Steady Boxing, LiveStrong, and Exercise is Medicine Exercise if Medicine. The Health Coach will ensure these programs operate successfully, within HIPPA compliance measures, and in accordance with policies, procedures, and established best practices.

  • Livestrong: a 12-week exercise program for those that have had a cancer diagnosis today or 30 years ago. This program addresses survivorship and improving quality of life.

  • Rock Steady Boxing: enables people with Parkinson’s Disease to fight their disease by providing non- contact boxing style fitness classes that improve quality of life, sense of efficacy and self-worth. Recent studies also suggest this intense program may be “neuro-protective†working to delay the progression of symptoms

  • Exercise is Medicine: a 12-week exercise program with rolling admission that provides adaptable exercise sessions with comprehensive education, instills confidence, and facilitates independence that will help patients develop positive lifestyle behaviors which can treat, prevent, and potentially reverse chronic diseases.

The Health Coach will ensure these programs operate successfully, within HIPPA compliance measures, and in accordance with policies, procedures, and established best practices.

Blood Pressure Self Monitoring: supports adults with hypertension in lowering and managing their blood pressure. The four-month program focuses on practicing home self-monitoring of blood pressure readings and individualized nutrition education for better blood pressure management.

Diabetes Prevention Program: Our year-long program engages participants in 25 sessions and focuses on small, measurable lifestyle goals. With group support and instruction from Lifestyle Coaches, participants learn how to incorporate healthier eating, physical activity, problem-solving and coping skills into their daily lives.

ESSENTIAL FUNCTIONS:

  1. Deliver and lead LiveStrong, and Rock Steady Boxing, Blood Pressure Self-Monitoring and Diabeters Prevention programs to association, YUSA, and PrismaHealth standards and work with branch staff to develop the overall operation of these programs.

  2. Support and conduct intake, coordination, delivery, evaluation and timely tracking for all EBHI programs.

  3. Support the growth and successful operation of EBHI programming for the association.

  4. Facilitate and strengthen community engagement in the service areas of our branch to expand and

    enhance the delivery of all EBHI programs.

  5. Track all data and provide necessary reports in a timely manner utilizing appropriate software.

  6. Serve as spokesperson and ambassador of EBHI programming. Maintain positive public relations with

    community and participants and always represent the best interests of the YMCA.

  7. Work closely with the Fitness Director and Community Health Coordinator to ensure programs and

    impact are promoted to our membership, participants, stakeholders, partners, and communities.

  8. Maintain open communication and work proactively to build strong relationships with all staff.

  9. Assist in planning special events that will create a positive image of the Foothills Area YMCA in our

    community.

  10. Follows and performs YMCA group exercise, fitness coach, and personal trainer policies and essential

    functions.

  11. Performs other duties as assigned.

QUALIFICATIONS:

  • Bachelor’s degree in exercise science or related field preferred

  • Must complete any program certifications within 60 days of hire

  • Must hold current NCCA personal training certification or higher, preferably ACSM for certain programs

  • Must hold current CPR/AED, First Aid, Child Abuse Prevention certification within 60 days of hire

  • Excellent customer service skills

  • Computer skills involved Microsoft Office and email

  • Ability to perform under stress and reach deadlines

  • Excellent time management skills

  • Personal passion for health and wellness

  • Knowledge and interest in public health/community integrated health and evidence-based initiatives

  • Ability to develop positive, authentic relationships with people from different backgrounds.
