The PCA Supervisor is responsible for field operations and supervision of the implementation of care and treatment plans, therapeutic activities for individuals as directed by licensed practitioners. Participates in the development of treatment places, program functions as well as policies and procedures. Responsible for the supervision of assigned caregivers (CNA's, PCA's and HHA's) to assure performance of tasks, functions and responsibilities necessary to assure appropriate individual care plan and active treatment is offered in a clean, safe and secure environment, in accordance with federal, state and unit policies and procedures. Provides supervision, counseling and evaluation of caregivers (CNA's, PCA's and HHA's) on assigned case.

Qualifications and Education requirements 

Highschool education or its equivalent; 

1 full year of CNA/HHA or PCA experience required; 

Home Health Aide training, Adult Residential License, Direct Care worker certification or CNA license required. 

Display tact, good judgement, interest in people and maturity in dealing with the frail, ill or geriatric population. 

Display flexibility, cooperation and dependability in fulfilling responsibilities. 

Duties and Responsibilities 

A. Will maintain all agency and state required supervisor visits. 

B. Conducts meet and greets, first visit shadowing and random field visits when necessary. 

C. Serves as the initial point of contact for all caregivers. 

D. Orients and trains staff to ensure best practice and standards of care are met. 

E. Develop, implement and train in best practices. 

F. Coordinates caregivers' daily assignments. 

G. Observes, provides and participates in monthly, quarterly and annual competencies and skill 

evaluations. 

H. Identify and communicate safety concerns. 

Stays current on best practices along with other materials provided Director of Nursing or Operations Manager regarding field operations. 

Observes all safety rules and applies accident prevention principles while performing duties; 

K. Delegate responsibilities and direction to caregivers. 

L. Communicates and advocates problem-solving skills as needs arise. 

M. Observes and documents patient status and reports any issues or complaints to the designated 

patient care coordinator. 

N. Supervises caregivers to ensure compliance and safety to all patients and caregivers.

O. Extensive knowledge of home care processes and procedures. 

P. Resolves patient requests, questions and initiates follow-up for concerns and provides 

'information to designated patient care coordinator. 

Q. Facilitates interdisciplinary communication and acts as a staff liaison to provide necessary 

guidance. 

R. Works in cooperation with administration and clinical staff to implement the company's mission 

and ensure provision of quality care. 

S. Completes all necessary forms and or data entry for consumers' case record, other management 

information and other written reports as required. 

T. Assists with the coordination, evaluation and planning of home care services. 

U. Complies with HIPAA privacy and security regulations that govern disclosure of protected health 

information. 

V. Ability to supervise and train employees, to include the execution of the care plan and 

compliance with organization, state and federal regulations. 

W. Assure client activities, schedules and Care plans are followed and conflicts are addressed. X. Conducts care audits to ensure appropriate staff and client interaction. 

Y. Ensures that health and personal care needs of the client are met by the assigned caregiver. 

Z. To ensure safe, adequate care to assess relationships and to determine whether goals are being 

met. 

A. Other duties as assigned. 

Working Conditions and Physical Effort: 

Supervision is provided at the residence of assigned clients. 

There may be some physical activity required. 

Some heavy lifting may be required.
