Position: Caregiver-CNA Dept./Division: Operations Reports To: Resident Care Director

Position Summary

The primary purpose of your job position assists residents in the community with all activities of daily care and

services in accordance with the resident’s plan.

Essential Functions

General Care

· Provide assistance and general resident care, including, but not limited to: social interaction, meal

assistance, bathing, toileting, and housekeeping

· Follow care plan, including monitoring resident inputs, outputs, vitals, behavior, safety concerns and any

other changes

· Upon request, transport residents to and from required appointments

· As needed, be an active participant in life enrichment activities

Documentation

· Contribute to employee communication log to ensure smooth transition of necessary information from

one shift to the other

· Document all inputs, outputs, vitals, behavior, and/or safety as needed in the client file

Minimum Qualifications

· Successful completion of a Certified Nursing Assistant program in the State of employment

· Current CPR/ First Aid certification (if applicable)

Preferred Qualifications

· Previous experience working in a memory care or assisted living community

Physical Working Requirements

· Ability to travel using personal vehicle, including at night

· Able to do occasional lifting of up to 50 lbs.

· Able to work flexible schedule, including evenings and/or weekends

· Team Member is subject to outside environment conditions occasionally, inside conditions frequently,

with protection from weather conditions but not necessarily from temperature change

· Tasks assigned to this position may involve potential and/or direct exposure to blood, body fluids,

infectious diseases, air contaminants, and hazardous chemicals

· Must be able to read, write, speak and understand English
