Your Role in the Company

Identifies, manages, and develops lead sources for Health Services Center or multiple Health Services nursing centers within an assigned market.  Conducts on-site clinical assessments of potential patients and determines appropriateness for admissions. Facilitates all aspects of a smooth transfer/admission process. Identifies, develops and maintains referral relationships and budgeted census.

Qualifications:

EDUCATION:

  • Graduate of an accredited school of nursing or therapy is preferred

LICENSES/CERTIFICATION: 

  • Valid clinical license in the state employed
  • Valid driver’s license.

Experience

  • A minimum of two years clinical experience, of which two years were in a long-term care environment.
  • Customer services, sales and/or marketing experience preferred.

 

Essential Functions of the Position

  • Receives leads from providers, facilities, or any customer source, and converts appropriate leads to drive census.
  • Manages and coordinates leads from key hospitals in the market.
  • Coordinates and communicates closely with internal Admissions person and facility Team.
  • In conjunction with facility requirements/abilities, clinically accesses patients to determine if the patient meets admission criteria.
  • Contacts potential lead as a representative of Health Services facilities. Assists with any questions from lead and/or family members.
  • Obtains the necessary medical and financial information to qualify patients for admission.
  • Facilitates patient placement based on clinical services needed, bed availability, patient/family preference, geographic location, etc.
  • Troubleshoots areas of admission, public relations, pre-certification and physician follow-up in conjunction with facility.
  • Conducts need assessments.
  • As necessary, may coordinate community education and events for hospitals, physicians and other referral sources.

KNOWLEDGE/SKILLS/ABILITIES:

  • Skilled in clinical admissions assessment.
  • Ability and willingness to travel frequently
  • Ability to work flexible hours.
  • Ability to work independently with minimum daily direction, and to self-motivate.
  • Ability to communicate effectively with patients and their families, physicians, hospital administrators, and at all levels of the organization.
  • Knowledge of, or ability to learn, reimbursement programs.
  • Ability to conduct oneself in a mature and professional manner.
