• Drug Enforcement Administration certification
• COVID vaccine card or exemption letter
• ANCC Certificate
• Searching for a physician who is board-eligible or board-certified in internal medicine, family medicine, or emergency medicine and who is experienced in primary care to provide on-site services to the male population at this facility. Full-time services will also include spending ½ day/week at the male institution across the street (Ross Correctional Institution). Schedule is weekdays - no evenings, weekends, or holidays. Assignment may last at least 6 months and eventually lead to permanent hire if there is mutual good fit. Both a phone interview and an on-site meeting/tour at Chillicothe Correctional Institution will be required prior to offer decision.

• The following credentialing documents must be submitted: a comprehensive resume that includes education (with location and year of graduation), training, and complete work history; and Ohio license; healthcare-professional level BLS/CPR certification and board certification. A Drug Enforcement Administration certification should also be submitted, if available, as it will be required for the assignment. If an offer is extended, drug and TB test documents will need to be supplied by vendor for review by hiring manager at least 48 hours prior to start, or the start date may be postponed.
