We’re seeking a compassionate, reliable CNA to deliver high-quality, resident-centered care. You’ll partner with nurses and the interdisciplinary team to support comfort, safety, dignity, and great outcomes.
What You’ll Do
Personal Care & ADLs: Provide bathing, dressing, grooming, oral/denture care, toileting, peri-care, and incontinence care while preserving dignity and privacy.
Mobility & Safety: Assist with safe transfers/ambulation using gait belts and lifts (Hoyer/Sit-to-Stand); follow fall-prevention/elopement protocols; turn/position residents per schedule to protect skin.
Nutrition & Hydration: Set up meals, assist with feeding, follow texture-modified diets and thickened liquids, observe aspiration precautions, and record intake/output.
Skin Integrity & Comfort: Perform routine skin checks; off-load heels, apply barrier creams per care plan; promptly report redness, breakdown, or pain.
Vital Signs & Monitoring: Obtain/document vitals, weights, and (if trained/authorized) blood glucose; observe and report condition changes to the nurse immediately.
Infection Control: Practice hand hygiene and PPE; follow standard/contact/droplet precautions; clean/disinfect equipment and high-touch areas after use.
Documentation: Chart ADLs, I/O, vitals, positioning, and care delivered in the EMR accurately and on time.
Resident Experience: Respond quickly to call lights; provide companionship and dementia-friendly redirection; support participation in activities.
Teamwork & Handoffs: Join shift report/huddles; follow individualized care plans; assist with admissions/returns (orientation, belongings inventory) and transport to therapy/appointments.
Unit Readiness: Make beds, change linens, tidy rooms, and stock supplies for a safe, organized environment.
Restorative Support: Assist with range-of-motion, ambulation, and other restorative nursing goals as directed.
Medication Reminders: Provide reminders per care plan and policy (no medication administration unless separately credentialed).
Qualifications
Active Texas CNA certification in good standing
1+ year CNA experience in SNF/LTC preferred (new grads welcome)
Compassionate, patient demeanor; clear, professional communication and teamwork
Ability to follow written/oral directions and individualized care plans
Able to stand/walk most of the shift and safely assist with lifts/transfers; reliable attendance
Basic EMR familiarity (training provided); meets all pre-employment/health requirements
