Job Summary

Odyssey House Louisiana, Inc. is seeking an experienced full-time Case Manager that will provide intensive case management services to participants in Community Supportive Services-Permanent Supportive Housing. The Case Manager is responsible for determining eligibility for chronically homeless individuals to participate in the OHL Permanent Supportive Housing Program (PSH). The Case Manager must provide Community Psychiatric Support Treatment (CPST); Psychosocial Rehabilitation Services (PSR) and Crisis Intervention to program participants.  The Case Manager provides tenant-based rental and operating assistance. The Case Manager must negotiate LA Medicaid systems. The Case Manager will also be responsible for documenting case notes and programmatic activities, which are pertinent to LA Medicaid systems.  The Case Manager is responsible for monitoring clients’ progress and needs, discharge plans, and tracking participants who have exited the program. The Case Manager is responsible for entering data into the agency’s assigned data bases and prepares weekly, monthly, quarterly and annual reports reflecting such activity, as indicated. The Case Manager is responsible for marketing the PSH program, recruiting perspective participants, and attending all pertinent trainings.

 

Responsibilities and Duties

·        Provides case management to approximately 25 participants as assigned. Case management will involve: screening, intake, assessment, service plan development, monitoring, linkage to appropriate community resources, follow up, appropriate discharge, tracking, etc…

·        Assists participants with completing lease agreements and obtaining other supportive

·        documents such as: identification, social security card, etc… as required by leasing agents/property.

·        Assess clients’ Medicaid status and follow agency’s policies and procedures to obtain and maintain Medicaid status.

·        Schedules and performs efficient client office and home visits that are geared towards CPST; PSR and Crisis Intervention Services.

·        Enters participant’s information into the appropriate agency’s database(s) in a timely matter and updates it as needed.

·        Prepares and submits reports to Project Manager in a timely manner.

·        Maintains up to date information regarding tenant occupancy, move outs, and vacancies.

·        Facilitates and coordinates supportive activities with other program staff including employment assistance activities, such as job readiness training, resume preparation,       interview practice, attending job fairs and networking with perspective employers, etc…

·        Conducts quality assurance program, self-monitoring plan as assigned.

·        Establishes and maintains collaborative working relationships with homeless shelters and community resources.

·        Attends appropriate coalition and other community resource meetings.

·        Attends training and workshops as they come available.

·        Develops promotional strategies to inform the community about the program including community presentations, print material, and related activities.

·        Must be able to work a flexible schedule.


Qualifications and Skills

Required:

·        At least 3 years adult case management experience

·        Ability to work with homeless individuals of diverse backgrounds and disabilities including physical, mental, substance abuse, HIV/AIDS, etc…

·        Outstanding communication and organizational skills

·        Ability to perform daily duties to achieve excepted outcomes and performance measures with minimal supervision

·        Ability to travel to various sites daily

Preferred:

·        Bachelor’s Degree in social work or behavioral science

·        Experience in LA Behavioral Health Partnership Billing

·        Experience in providing Community Psychiatric Treatment; Psychosocial Rehabilitation and Crisis Intervention
