Case Manager/Employee Health Nurse, RN (Salaried/FullTime - RN License Required)

The Case Manager will perform all functions related to ensuring that patients meet proper admission criteria, sending clinicals, interfacing with the provider team to ensure that documentation is appropriate to the condition and for all discharge planning activity.  As the census at NCCH is typically low (usually no more than 3-5 patients) this position also performs employee health functions which include ensuring immunization and/or titer status, administering workplace injury processes, etc.  Medical floor and ED nursing experience preferred. 

Certificates, Licenses, Registrations:

Current, unrestricted Registered Nurse, license in the State of Arizona required. 

Current, Basic Life Support (BLS) Certificate (American Heart Association), required – must maintain current certification and recertify prior to expiration date. 

Advanced Cardiac Life Support (ACLS) Certification (American Heart Association), required – must maintain current certification and recertify prior to expiration date.

Commission for Case Management Certification (CCM) preferred.
