Case Manager - Indigenous *DESIGNATED (External)

About Our Opportunity:

Reporting to the Manager, Housing First, the Case Manager works within the Housing First program of NECHC. The Case Manager provides ongoing support to individuals to exit homelessness as they secure and establish permanent housing at the Overlook. Working within the Case Management model, they will support these individuals in creating personalized plans and goals. Case Managers will meet on a regular basis with the individuals on their caseload and provide the necessary support for the participant to maintain housing stability and meet their personal goals.


Key responsibilities for this position include the following, but are not limited to:

  • Developing and maintaining individualized recovery plans based on Housing First and Harm Reduction principles, monitoring progress towards goals.
  • Managing a caseload of 15-30 individuals, providing coverage as needed and ensuring housing stability in the Overlook.
  • Assisting with housing searches, applications, and viewings for individuals exiting homelessness or at risk of losing housing.
  • Providing support in the home and community, connecting participants with resources, and facilitating transportation to case plan related activities.\
  • Maintaining accurate documentation, ensure timely communication with service providers, and advocate for participants when necessary.
  • Participating in team meetings, community events, and internal organizational development activities to improve service outcomes.


About You:

This may be the right opportunity for you if you have post-secondary education in Human Services and/or other related fields such as Psychology or Social Work and 2 years’ experience working with participants who have lived experience and are at risk.

Other required knowledge, skills and abilities include:

  • Having a strong understanding of the systemic issues of homelessness
  • Must have access to a personal motor vehicle and a valid driver’s license to transport participants to and from services in the community.
  • Having good communication, documentation and interpersonal skills, and are self-motivated and able to work with minimal supervision.
  • Having a good understanding of the principles of Housing First and Harm Reduction programming is considered an asset.

Additional Information

Case Manager will work 37.5 hours a week and can include irregular scheduled hours, evening and weekend shift work. Schedules are determined by the Manager, Housing First.

If successful, you must complete a Vulnerable Sector Check and Criminal Record Check before beginning employment as these are a requirement for all NECHC employees working directly with clients, participants, and patients.
