Case Manager
SUMMARY:
Counsel and aide individual clients and families in crisis, assisting them in attaining their highest potential level of self-sufficiency.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
1. Conduct screenings to identify patient needs though initial human service needs assessments.
2. Review supplementary patient information such as employment, medical records, mental health records, psychological testing, and school reports to ensure continuity of care and improved health outcomes.
3. Utilizes information gathered during assessment process and through collaboration with other members of the treatment team, engage the client in the development of a comprehensive treatment/care plan designed to outline goals and objectives to be achieved.
4. Partner with ARMS’ Licensed Clinical Social Worker (LICSW) and Licensed Mental Health Counselor (LMHC) to obtain mainstream entitlements.
5. Coordinate the development of a comprehensive progress report and submit case for review based on agency protocol.
6. Coordinate warm hand-offs internal LICSW/LMHC or psychiatrist as well as any external partners providing MH and SUD services.
7. Counsel clients individually, in family, or in other small group settings regarding plans for meeting needs and aids clients to mobilize their inner capacities and environmental resources to improve client’s social functioning.
8. Determine client’s eligibility for financial, housing, medical and other community resources as well as assist with enrollment processes as requested.
9. Coordinate and provide client with transportation resources available to assist clients in attending agency or community-based appointments.
10.Collaborate with client to identify barriers to achieving self-sufficiency and advocate on the client’s behalf to obtain resources necessary to meet their needs.
11.Conduct and document follow-up activities to determine status of referrals.
12.Maintain required documentation, charting and reports.
13.Attend scheduled one-on-one sessions with supervisor, departmental meeting and organizational-wide town hall meetings.
14.Perform other duties as assigned.
EDUCATION AND EXPERIENCE:
Bachelor’s degree in social services or related field.
Minimum of 1 year of case management experience.
Must have a valid Alabama Driver’s License, insurable driving record and dependable transportation.
