Case Manager
Case Managers are responsible for providing program intakes and case management services in the areas of, but not limited to independent living skills, housing stabilization, money management, community integration, employment linkage, benefits establishment, linkage to community providers for substance abuse, primary and mental health care, and all other services needed to assist clients in reaching their treatment plan goals.

- Serves and protects Christ-Centered Ministries / Restoration Reentry Residence practices by adhering to professional standards, policies and procedures, federal, state and local requirements.
- Maintains accurate documentation of service objectives and outcomes as well as other services in accordance with Federal, State, County and Christ-Centered Ministries / Restoration Reentry Residence guidelines.
- Secures client information and maintains client confidence by completing and safeguarding data. keeping client information confidential and in compliance with HIPAA standards.
- Maintains safe, secure, and healthy work environment by establishing and following standards and procedures, complying with legal regulations including acting as a mandated reporter.
Responsibilities
- Conduct crisis and risk assessments in consultation with supervisor/ team or LCSW.
- Provide crisis intervention services focused on enhancing the clients’ ability to independently problem solve, utilize effective coping skills, and manage and self-coordinate own care.
- Utilize evidence-based practices in service delivery such as intensive case management, Motivational Interviewing, Harm Reduction, Critical Time Intervention and Housing First practices.
- Maintain documentation standards as set forth by the program contract and CCM policies.
- Complete progress notes on every face to face/ telephone contact with client or collateral contact by the next business day at 5pm.
- Develop Individualized Housing and Service Plans (IHSP) for each participant based on elements from the comprehensive assessment.
- Identify permanent housing options for program participants.
- Determine and recommend to supervisor the level and duration of financial assistance for eligible participants and submit all required documentation to supervisor for approval.
- Manage and track financial assistance funds by using updated technological and tracking tools.
- Collaboratively work with Housing Locators to find permanent housing options for program participants and whenever necessary to mitigate issues or advocate for participants.
- Other duties as assigned.
Qualifications
- Bachelor's Degree in social work, social services, public administration, public health.
- At least two years of experience in benefits advocacy, healthcare/benefits administration a plus.
- At least two years of case management experience connecting clients to resources.
- Understanding and demonstrated ability to navigate governmental benefit programs preferred.
- At least two years of experience providing benefits to homeless individuals.
- Knowledge of crisis intervention, Part-R, CPR and First Aid.
- Knowledge of various Applications and Devices including but not limited to: Microsoft Office applications, Dropbox, Computers, iPads, mobile devices, etc.
- Must have a valid CA driver license, have reliable transportation and a good driving record.
- Bilingual (Spanish/English) is a plus.

Physical Requirements
There is some walking, bending, and kneeling. Must be able to participate in moderate lifting of at least 30 lbs. or more (suitcases, packages, etc.). This position must be able to support client weight in case of emergency situation or client disability requiring assistance in walking, etc.
Workplace Environment
There is a risk of exposure to bodily fluids infections, contagious / blood borne diseases, and chemicals (cleaning agents), an exposed risk to varying, unpredictable situations (i.e., client behavior).
